Job Summary
The Transplant Program Coordinator plays a vital role in supporting the daily operations, regulatory compliance, and patient coordination within a multi-organ transplant program. This position requires strong organizational skills, attention to detail, and the ability to manage multiple priorities in a fast-paced healthcare environment.
Essential functions
+ Manages and coordinates referrals from patients, providers and dialysis units including pre-screening for transplant evaluation eligibility.
+ Schedules evaluation/follow-up appointments and coordinates testing for potential transplant recipients/donors. Assists/follows through on all appointments.
+ Maintains patient evaluation schedule (in written or electronic) form, for the multidisciplinary team. Responsible to monitor/maintain physician/nurse/patient electronic schedule and compile/tabulate daily statistics for budget reporting purposes.
+ Accurately and professionally complete outpatient registrations, ensuring all patient information is collected and verified, including insurance details from patient ID cards.
+ Schedules patient tests, labs, and procedures, while assisting with patient communication regarding testing plans and sending appointment reminders via phone or letter.
+ Answer and triage departmental phone calls, prioritizing, redirecting, and providing information while maintaining an extensive knowledge base to efficiently manage patient inquiries and ensure timely care.
+ Perform chart review to coordinate testing/appointments.
+ Manages clinic operations for the multidisciplinary team, ensuring patients are evaluated/assessed from all required regulatory team members in a timely and efficient manner
+ In coordination with Living Donor Coordinator, manages the entire donor process from initial questionnaire receipt through and including two years of follow-up after donation per regulatory requirements.
+ Manage outside records for team review and scans into EPIC for Transplant Team access. Assembles/Maintains/Organizes Patient Education Materials.
+ Orders education materials as needed.
+ Maintains inactive chart storage process including database management.
+ Coordinate monthly HLA mailers and follow up with patients and dialysis units Responsible for day to day office operations including but not limited to, calendar management, supply purchasing, Selection Committee documentation, etc.
+ Writes and sends correspondence to patients, providers, and dialysis units notifying them about their patients as required by regulatory agencies.
+ Support compliance with CMS, UNOS, TJC, and other regulatory agencies. Support quality improvement initiatives using PDCA, Kaizen, or GEMBA methodologies.
+ Additional secretarial/clerical functions or special projects as assigned by Program Leadership.
Qualifications
Required
+ High School Diploma
Preferred
+ Associate’s degree Associate and/or bachelor’s degree in business and/or education background.
+ Proficient in Corewell Health systems; Outlook, Kronos, Epic and MS; Word, Excel, PowerPoint, and Internet applications.
+ One year of experience in healthcare setting.
