    Job Summary and Qualifications - PT Days

    + Demonstrates the knowledge, skill, and coordination ability to provide guidance to surgical patients.

    + Partners with patients, families, physicians, interdisciplinary team members and community resources to provide well- coordinated, timely, compassionate interdisciplinary care.

    + Serves as an advocate for patients and family members regarding their needs and concerns about their diagnosis and treatment plan. Communicates with all members of the healthcare team, as appropriate, about patient/family needs and concerns.

    + Communicates regularly with patients and family members referred to the navigation service. Provide assistance with scheduling, coordination of treatments, requests for information and psychosocial support.

    + Supports patients and family members during decision-making processes. Provides emotional support during treatment and follow-up care for patients and caregivers.

    + Provides direction to patients with issues such as insurance, transportation, child care, financial resources, language barriers, etc. Facilitates access to appropriate resources to address patient concerns so they may focus on treatment and recovery.

    + Provides education and information to patients and family members to help make care seamless, continuous and comprehensive. Responds to requests for treatment information, side-effect management and community resources.

    + Collaborates with key staff in priority sites to support patient referral mechanisms and promote the Patient Navigation process.

    + Establishes relationships with area service providers and other healthcare networks to enhance service and referrals in the community.

    + Documents the patient navigation process for each patient assigned to the navigation service including assessment, plan of care, patient education and evaluation of the navigation process.

    + Promotes and educates physicians and other health professionals about the Navigator role and process of patient navigation.

    + Serves as a resource for community educational events such as health fairs, screenings, symposiums and lectures as well as staff education.

    + Systematically and continually performs the functions of assessing, planning, implementing and evaluating care according to the nursing process.

    + Initiates and performs on-going review of policies related to patient navigation services. Reviews, updates and rewrites policies as necessary to enhance professional practice.

    + Assists the Marketing Department in the development of educational and promotional materials.

    + Works with physician office prior to admission to prepare, anticipate and communicate individualized needs of the patient to enhance the hospital experience.

    + Communicated with physician office regarding discharge plan and communicate any changes of plan.

    + Formulates a teaching plan, based on identified patient learning needs and evaluate effectiveness of learning; family is included in teaching as appropriate from admission to discharge.

    + Completes follow up phone calls to all patients within one week of discharge.

    + Works with unit leaders to implement and maintain established department, unit and hospital policies.

    + Teaches patient education classes.

    + Round with physicians as requested.

    + Educate and mentor nurses and techs on the unit.

    + In coordination with office and OR staff, anticipate for unique patient needs i.e., bariatric bed, etc.

    + Supports and displays the facility ICARE values.

    + Performs other related functions as assigned.

    What qualifications you will need:

    + 3 to 5 years Orthopedic patient care experience. Orthopedic navigator experience preferred.

    + Basic Cardiac Life Support must be obtained within 30 days of employment start date

    + High School Graduate / GED

    + (CST) Certified Surgical Technologist, or (RN) Registered Nurse must be obtained within 30 days of employment start date
