Intensive Case Manager

Position Summary:
The Intensive Case Manager is responsible for providing comprehensive substance abuse assessment and case management services to formerly incarcerated clients of IMPACT program, in Queens, NY, who may have substance abuse barriers to employment by applying clinical judgement to determine appropriate treatment needs and levels of care.
Responsibilities:
+ Orients clients about program goals, objectives, rules and regulations, client’s obligations, client’s rights, and program operations.
+ Works cooperatively with other staff as a member of the interdisciplinary team.
+ Functions as a liaison/advocate to clients’ families and/or other agencies as needed.
+ Demonstrates appropriate treatment planning and clinical intervention.
+ Conduct thorough assessments and through a motivational interviewing process, develop individualized person-centered transition plans.
+ Determines the appropriateness of the level of care being currently provided by the treatment program for clients already in treatment, including relapse prevention.
+ Reviews the treatment history utilizing available resources.
+ Facilitates groups and seminars on Substance use topics.
+ Identifies available vacancies in appropriate treatment programs and makes appointments for initial interviews for clients, minimizing disruption to clients and family, where clinically appropriate.
+ Ensures the maintenance of accurate, complete, timely and high-quality client records and reporting of client information that comply with external regulatory standards and agency policy and procedure.
+ Participates in case conferences with other community-based service providers, including treating physicians, clinicians, Case Management staff, housing specialists, benefits, or entitlements specialists and/or other needed supports in the community.
+ Provides crisis intervention/management, generating referrals to Mental Health Services and other provider resources, as indicated.
+ Maintains exceptional recordkeeping and documents all communication and contact with participants and external partners in FedcapCARES.
Education and Experience:
+ Master's degree in Social Work or Mental Health Counseling or a relevant field, such as Psychology, Vocational Rehabilitation, or Occupational Rehabilitation or bachelor’s degree with Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
+ Must have strong writing skills; competencies in writing goal-based/person-centered treatment plans and progress notes as evidenced by successful completion of writing sample.
+ Bi-lingual (English/Spanish) preferred.
