As a Social Worker or Social Worker Designee at PortagePointe, you will be responsible fordirecting the social service to promote Elder independence in their home. This role will investigate, assess and plan interventions to help Elders cope with social, emotional, economic, and environmental difficulties. This position will nurture, sustain, and protect each Elder by providing the services needed to live a full life, in line with the Eden Philosophy.

• Demonstrates proficiency in verbal and written communication skills

• Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external

Position Requirements:

Associates or Bachelor’s Degree Required

Current Michigan Social Worker License, Licensed Practical Nurse or Registered Nurse Required

If not Social Worker, Social Work Designee certification required

Long Term Care Experience Preferred

Basic Life Support (BLS) Certification Required (Available Upon Hire)

Annual Influenza Vaccination or Declination Required
