• **Location:** Hybrid – In-Office Tues/Wed/Thurs, WFH Mon/Fri - **Must live close to Chicago, IL**
• **Schedule** : 6:45 AM – 3:30 PM
• **Start Date** : June 23, 2025
• **Pay Rate:** $40–$43/hr (based on experience, location, and education)
Are you a compassionate and skilled healthcare professional with a passion for patient-centered care and care coordination? Adecco Healthcare & Life Sciences is hiring a Hybrid Health Care Occupational & Case Manager for a dynamic opportunity supporting individuals with complex medical needs.
This is an ideal role for someone who thrives in a collaborative environment, has strong case management experience, and is looking for a balanced hybrid schedule!
• **About the Role:**
As a Health Care Occupational & Case Manager, you’ll work directly with patients, providers, and insurers to ensure seamless, high-quality care. You’ll assess needs, build care plans, and advocate for the best outcomes.
• **Key Responsibilities:**
• Conduct medical, psychological, and social assessments
• Develop personalized care plans with healthcare teams and patients
• Coordinate treatments, therapy, consults, and follow-ups
• Educate and support patients and families
• Monitor progress and adjust care plans accordingly
• Advocate for patient needs and ensure services align with healthcare regulations
• Document care thoroughly and accurately
• Stay informed on best practices and compliance standards
• **Qualifications:**
• RN, NP, or PA license (Must be licensed in IL)
• 2+ years of experience in occupational health, nursing, or related field
• Case management experience required; CCM certification preferred
• Strong knowledge of workers' comp, disability, industrial hygiene, and healthcare plans
• Proficient with computers and mobile technology
• Empathetic, solution-oriented, and highly organized

**Pay Details:** $43.00 per hour
