Clinical pharmacy is a key driver for reducing pharmacy costs and improving patient health outcomes. This position provides direct patient care blended with the opportunity to work with SLPG leadership to develop the role of population health pharmacy within SLPG. This position will have a blend of on-site clinical and administrative work and the opportunity for telework.

Uses evidence-based outcomes to improve medication therapy and chronic disease management for individuals covered by value-based programs in which SLPG participates. This is an excellent opportunity to provide patient care and to be in a position of leadership to shape the future of population health pharmacy within the clinically integrated network.

Establishes, implements, and evaluates medication management policies, procedures, clinical care, and data collection for patients under the care of SLPG practices. The clinical pharmacist will collaborate with physicians, other clinical pharmacists, and other clinical team members to deliver care while shaping out processes to improve the quality of care across a clinically integrated ambulatory environment.

JOB DUTIES AND RESPONSIBILITIES:

+ Conducts patient medication history review and formulates an individualized patient medication plan which includes lifestyle modifications and address barriers to adherence.

+ Provides an organized and comprehensive approach to consultations for medication management including indications, medication selection, medication dosage, access to medications, drug reactions, and drug interactions.

+ Leads initiatives to optimize medication use across the network such as reducing the use of high-cost, low value medication prescribing, coordinating medication conversions with prescribers and optimizing the use of the electronic health record.

+ Responsible for physician consultation and recommendations. Working with SLPG leadership in consultation with SLUHN pharmacy leadership, develops and maintains policies and procedures related to mediation management based upon his/her clinical experience in our ambulatory settings.

+ Serves as a resource and liaison for implementation of drug management initiatives across SLPG. Working with physician leadership, develops protocols and pilots them to initiate, adjust, monitor, and recommend drug therapy.

+ Functions as a direct-patient-care clinical pharmacist in these initiatives. Collects, analyzes, interprets, and forecasts prescribing habits based on current patterns from medication utilization reports.

+ Collaborates with clinical and administrative staff to determine need for process or system improvements related to medication utilization, including polypharmacy, opioid utilization, value-based prescribing, and optimization of medication therapy for select chronic conditions including heart failure, diabetes, hypertension, and COPD/asthma.

+ Maintains an active leadership role on select committees and workgroups identified by SLPG leadership.

+ Opportunities to lead project teams to implement process and/or system changes.

+ Working with physician leadership, develops protocols and pilots them to initiate, adjust, monitor, and recommend drug therapy. This includes use of the patient medical record, review of laboratory results and direct patient contact. Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.

+ Leads project teams to implement process and/or system changes.

+ Possesses excellent verbal and written communication skills; strong business analytical skills and abilities; strong project management and coordination ability; ability to function effectively as a team member.

+ Maintains confidentiality of all materials handled within the Network/ Entity as well as the proper release of information Complies with Network and departmental policies regarding issues of employee, patient and environmental safety and follows appropriate reporting requirements.

+ Demonstrates/models the Network’s Service Excellence Standards of Performance in interactions with all customers (internal and external). Demonstrates Performance Improvement in the following areas as appropriate: Clinical Care/Outcomes, Customer/Service Improvement, Operational System/Process, and Safety. Demonstrates financial responsibility and accountability through the effective and efficient use of resources in daily procedures, processes and practices. Complies with Network and departmental policies regarding attendance and dress code.

OTHER FUNCTIONS:

Other related duties as assigned.

PHYSICAL AND SENSORY REQUIREMENTS: Sits up to 7 hours per day; 1 hour at a time. Stand for up to 8 hours per day; 4 hours at a time. Walk up to 6 hours per day; 10 minutes at a time. Hearing as it relates to normal, high and low frequencies. Seeing as it relates to general, far, near, color and peripheral vision; normal depth perception.

POTENTIAL ON-THE-JOB RISKS:

MOST COMPLEX DUTY: Utilizing critical thinking skills to coordinate patient care and to interact with physicians, clinical staff and administrative leaders in the development and implementation of clinical pharmacy programs.

SUPERVISION (RECEIVED BY AND/OR GIVEN TO):

Direct Report to: Manager, Ambulatory Clinical Pharmacists for St. Luke's Physician Group.

COMMUNICATIONS: Excellent written and verbal communication skills required for contacts with providers and administration. The Ambulatory Clinical Pharmacist will regularly collaborate with medical staff and other network departments.

ADDITIONAL REQUIREMENTS:

+ Doctor of Pharmacy degree.

+ Proficient at working with Electronic Health Records Systems.

+ Working knowledge of the use of enterprise data warehouses and patient registries.

+ Computer skills including Microsoft Word, Office, Excel, and PowerPoint.

+ Travel across the approximately 10 counties in east-central PA and west NJ served by SLPG for meetings and to work with clinical staff is anticipated. Occasional longer distance travel for state or national meeting is possible.

+ Current driver’s license and vehicle access required.

+ Works independently to meet expected deadlines for reports, projects and presentations.

+ Must possess excellent organizational skills.

+ Understanding of the principles of clinical integration and population health management.

+ Active Pharmacy license in Pennsylvania by start date; eligible for licensure in New Jersey if needed. National Provider Identifier (NPI) is required.

MINIMUM QUALIFICATIONS

EDUCATION:

Doctor of Pharmacy degree is required.

TRAINING AND EXPERIENCE:

+ 3-5 years of clinical experience is required.

+ Successful completion of a PGY2 pharmacy residency in ambulatory care or equivalent clinical experience in direct patient care is required.

+ Board of Pharmacy Specialties certification preferred.

+ Excellent communication and project management skills including process improvement preferred.

+ Experience with the development of clinical pharmacy programs within a clinically integrated network or an accountable care organization.

WORK SCHEDULE: Part-time available. Part-time: 20-hour work week. Flexible schedule as needed.
