    Nature of Work

     

    Provides lead direction and training to staff members and performs comprehensive case management of long-term care services for seniors within their homes and at the City's Senior Program Centers.

     

    Environmental Factors

     

    Work is performed in a standard office environment, in various community facilities, and in client homes where various hazards may exist. Exposure to all weather and traffic conditions when traveling.

     

    Physical Factors

     

    Light physical demands; some lifting and moving of supplies and equipment. Sufficient clarity of speech, hearing, or other communication capabilities, vision or other powers of observation, to effectively and efficiently perform essential job functions.

     

    Work Situation Factors

     

    Position involves competing demands, performing multiple tasks, working to deadlines, frequent work at night, beyond normal business hours, and responding to customer issues. Regular attendance is an essential function of this job to ensure continuity of services. Position is subject subject to drug testing in accordance with applicable State and Federal regulations and City of Las Cruces policies.

    Duties and Responsibilities

    + Complete all Initial Assessments for Long Term Care Services such as Meals on Wheel's, Homecare Services, Respite Services and Grandparent Program; supports any congregate clients that need assistance with advocacy, assessments, reassessments, referrals, community assistance, and filling out applications for social programs available.

    + Oversees the Respite Care Program that provides respite to caregivers caring for their loved ones who have been diagnosed with Dementia or Alzheimer's Disease; prepares the schedules for respite services and coordinates ensure that clients needs are being met.

    + Maintains regular communication with internal staff, family members, and professional agencies to access additional community services; conducts field visits and makes visual assessments of client's physical surroundings to assess for various hazards.

    + Maintains all client documentation, written assessment tools, case notes, family correspondence, referrals, legal reports, and various other documents as required; coordinates information and referrals with other social service agencies and health care providers to obtain necessary services to protect both health and safety of clients.

    + Maintains on-going community networking efforts with home health care agencies, hospitals, doctors, and public and private social service agencies to ensure available services are provided to clients; develops and coordinates special projects for in-home services including planning, promoting, implementing, and evaluating special activities and events.

    + Meets regularly with staff to review work in progress, discuss and resolve administrative, workload, and technical issues, and to prioritize and assign tasks and projects; evaluates and analyzes functions, resources, processes, and procedures to identify issues and recommend changes for improvement.

    + Manages staff through proper delegation and work supervision to provide appropriate levels of service and resources; ensures adherence to established policies and procedures to remain in compliance with local, state, and federal regulations.

    + Collects, gathers, and analyzes data to prepare and present various reports providing accurate and timely information related to assigned functions and duties; develops and presents information in clear and easily used formats; maintains regulatory compliance by keeping current on industry trends, regulations, and issues; identifies areas of concern for management for review and potential action.

    + Responds to and resolves various complaints, issues, inquiries, and problems as authorized and in accordance with established policies and procedures.

    + Assists in handling complex cases, serves as lead to co-workers, trains other employees, and serves as resource to other staff as back -up supervisor.

    DISTINGUISHING CHARACTERISTICS

    This is the senior level class in the Human Services Case Manager series; employees at this level perform a multitude of technical, complex and/or specialized duties which may include additional responsibility for checking completed work for compliance with codes and regulations, handling of complex cases and serving as lead to other workers, train other employees, serve as resource to other staff and as back-up to supervisor.

    Minimum Qualifications

    Bachelor's degree in Social Work, Nursing, or related field AND three (3) years of experience in casework management and community service programs for the elderly and disabled communities. A combination of education, experience, and training may be applied in accordance with City of Las Cruces policy.

     

    Licenses/Certification(s)

     

    Valid driver’s license is required. Position requires an acceptable driving record in accordance with City of Las Cruces policies. Case Management Certification and Bilingual skills (English/Spanish) may be preferred.

     

    Knowledge, Skills, and Abilities

     

    Knowledge of: principles, practices, techniques, activities, rules, and regulations related to elder care issues, especially with high risk clients, and eligibility for programs that provide social services in the community; case management processes for Long Term Care services including, but not limited to, eligibility for State and Federal benefits and insurance, Developmentally Delayed (DD) Waiver, Disabled and Elderly (D & E) Waiver, Supplemental Security Income (SSI) and Social Security Disability (SSDI), Medicare Part A & B, Medicaid, Medically Fragile, Durable Medical Equipment, Personal Care (PC) Option, and other community programs and benefits; ; current principles of record keeping and records management; principles and practices of effective research methods and data analysis, customer service, and problem resolution techniques; occupational and environmental safety and health hazards, and safety practices; safe and effective use and maintenance of related tools, materials, and equipment; current methods and standards for preparing effective business correspondence; correct English usage, grammar, composition, spelling, punctuation and vocabulary; business and personal computers and related software applications; City organization, operations, policies, and procedures to effectively perform the required functions and duties of the position.

     

    Ability to: perform a variety of duties and responsibilities and assess and prioritize multiple tasks, projects, and demands to meet deadlines; ensure appropriate levels of customer service to achieve expectations and meet objectives; read, understand, and assure compliance with a variety of policies, procedures, rules, standards, and regulations governing related activities and functions; collect applicable information and maintain accurate and timely records; maintain confidentiality of information and data; communicate effectively orally and in writing and use interpersonal skills to sufficiently exchange or convey information and receive work direction; prepare and present accurate and reliable information and reports; establish and maintain effective and appropriate working relationships with employees, other agencies, and the public; safely and effectively operate relevant tools, equipment, and motorized vehicles; navigate across even and uneven surfaces; timely and accurately enter data and update records using various mediums and formats; take initiative and exercise sound independent judgment within established procedural guidelines and carry out primary responsibilities in accordance with all governing regulations, statutes, and procedures to ensure achievement of goals and objectives.

     

    Skills in: client assessment and development of treatment plans and case management; reading, understanding, and applying relevant rules, ordinances, codes, regulations, policies, and procedures; effectively managing assignments; responding to inquiries timely and within the scope of delegated authority; responding appropriately, maintaining objectivity and freedom from prejudice, and exercising sound judgment and understanding in all interactions; effectively working independently, or as a team member, in various environments with changing priorities; preparing and presenting various reports, documents, forms, and correspondence; researching information, analyzing data, maintaining accurate records, and updating information in various mediums and formats; operating a personal computer with installed generic and specialized software; preparing and presenting information in a clear and concise manner; demonstrating appropriate and effective interpersonal communication and conflict resolution skills; performing highly effective support in assigned areas
