    Human Services Counselor II

     

    This is an office-based position within the Maternal & Child Health Home Visiting Division, under the supervision of the Human Services Program Supervisor II, Healthy Families. The Office Based Human Service Counselor II functions as part of the Coordinated Intake & Referral Team, which is a collaboration between the Healthy Families Program and the Healthy Start Coalition. This position completes initial interviews with pregnant women to determine which home visiting programs they are eligible for and which programs will best fit their needs and preferences. This position then completes all the necessary paper and computer documentation to finish the process. This position will work within a designated prenatal physician’s office within Pinellas County and will be required to travel to attend meetings, attend trainings or cover another physician’s office. This is a professional position that requires excellent verbal and written communication skills and the applicant must demonstrate the ability to work with diverse ethnic, and socio-economic populations. This position requires the ability to work well independently and work at part of a team. The ability to multi-task and prioritize tasks is essential. The ability to work with a variety of personalities is a must.

    Your Specific Responsibilities:

    Assessment and Eligibility

     

    + Provides office-based intake and referral services in a clinic setting including completion of the initial intake, assessment and linkages to services.

    + Determines participant needs through interviews during prenatal visits.

    + Accurately reviews Healthy Start Prenatal Risk Screening instrument on all OB patients seen at clinic.

    + Explains the benefits and purpose of available home visiting services.

    + Ensures required efforts are made when speaking to clients, to ensure all pregnant women are outreached.

    + Treats all with dignity, respect, courtesy and sensitivity.

    + Complies with HIPAA requirements regarding confidentiality of protected health information.

    + Educate and improve families’ knowledge and understanding of available resources and services that support the well-being of the family.

    + Facilitates family’s choice of home visiting and other programs based on their needs and preferences and the information identified through the Healthy Start screen or other screening or assessment tools.

    + Develop and maintain a comprehensive understanding of maternal/familial/child health issues that are risk factors during pregnancy and early childhood. This includes medical, family violence, mental health, father, and co-parenting services in the community.

    + Develop and maintain positive working relationships with partner programs and medical providers.

    + Work with supervisor and providers to support, train and resolve issues with Healthy Start screenings.

    + Assesses client and family mental health needs and identify trauma and toxic stressors; refer to appropriate programs and services.

    + Generate necessary correspondence in compliance with program guidelines and expected communications with providers.

    + Maintain accurate records and document actions taken, including tracking of referrals and partner program enrollment outcomes in a timely manner and following program guidelines.

    + Develop and maintain excellent working knowledge of electronic record system and Healthy Start Standards and DOH, Healthy Families Program guidelines.

    + Prepare and submit appropriate reports, accurately and on a timely basis.

    + Provide community outreach and education on the Healthy Start Coalition and its programs and partner home visiting programs through meetings, presentations, health/social service/back to school and other events/fairs.

    + Demonstrates ability to function effectively as part of a team by: providing prompt communication to team members regarding consumer issues

    + Displays a willingness to cover duties for other team members when necessary.

    + Incorporates knowledge of risk factors into daily activities.

    + Coordinates efforts with the Healthy Start Coalition, the Coordinated Intake and Referral program, and the Department of Health, Healthy Families Program.

    Required Knowledge, Skills, and Abilities:

    + Effectively applies basic counseling skills with families such as developing rapport, establishing & maintaining effective supportive professional boundaries, using active listening & reflection. Knowledge of maternal and child health, infant mental health, child development and trauma informed care practices.

    + Excellent written and verbal communication skills.

    + Effectively utilize Microsoft office programs and possess basic computer skills.

    + Must be able to work independently and effectively.

    + Can demonstrate sound judgment, dependability, and good teamwork and communication. skills. Must be able to handle sensitive and confidential situations with discretion.

    + Has a tactful, pleasant, and friendly approach in dealing with families, staff, guests, and individuals within the community.

    + Has the ability to work flexible hours as needed, including evenings/Saturday.

    + Can travel within Pinellas County and state as needed; must be flexible with respect to daily work location.

    + Must have a valid Florida driver’s license and the ongoing use of a reliable automobile.

    + Travel to state/national conference(s) may be required.

    Qualifications:

    Minimum-

     

    + Computer proficient, including Microsoft Office Suite, and internet.

    + Reliable transportation daily to get to multi-site work locations, trainings, and meetings.

    + Ability to work a flexible schedule, including evenings and Saturdays.

     

    Preferred-

     

    + A bachelor's degree with a minimum of two year's experience working with pregnant women and/or families with children aged zero to five years old OR;

    + An associate's degree and a minimum of three years experience working with pregnant women and/or families with children aged zero to five years old.

    + Bilingual in English and Spanish with speaking, reading and written fluency.
