Job Summary Comfort Care Hospice Aide - Assess patients needs and renders appropriate care:
Qualifications:
Certified Nursing Assistant - Mississippi nurse aide registry.
Organizational Expectations:
Provides age appropriate personal care according to care plan. - Accurately obtains and documents vital signs, (temperature, pulse, & respiration) each visit. - Consistently demonstrates proper administration of: Skin Care Patient bath (partial/complete, sponge, bed bath, shower) Hair care/shampoo Nail care Shave Foot care - Demonstrates adequate skill in changing bed linens (occupied/unoccupied) - Assists with ADL’s as outlined in care plan. - Assists with range of motion exercises as instructed in care plan. - Transfers patient safely and appropriately. - Provides light housekeeping and food preparation as needed and in compliance. - Knows basic care of patient with I.V., N/G tube, O2, Ostomy. - Competent in use of, or assisting patient with, equipment: wheelchair, Hoyer, bedside commode chair, etc. Documentation/Reporting - Completes documentation of patient care accurately and delivers to assigned personnel according to agency policy. - Reports to Team Leader/RN any patient changes/problems timely. Functions as a member of the healthcare team in delivering mission-driven, quality patient care: - Follows patient schedule, notifies Team Leader in timely manner of delays. - Functions effectively within the scope of practice for nurse aide to ensure patient is receiving quality care. - Communicates with Team Leader/RN to schedule routine supervisory visits. Completes supervisory visits as scheduled. - Attendance is consistent with accepted agency standards. - Maintains agency objectives, continuous quality improvement, safety, environmental and infection control standards. Demonstrates knowledge of standards of care. Willingly acquires new knowledge and skills needed. - Attends all in-services, education programs, and scheduled meetings as outline by the Director/Team Leader. - Maintains and demonstrates a constant awareness of patient’s rights and ethical aspects of aide duties. - Consistently demonstrates proper judgement in delivering patient care within the scope of the nurse aide position. Service: Demonstrates actions which create a positive experience for the customer. - Maintains confidentiality, protects patient’s modesty. - Observes Standard Precautions. - Communicates with patient appropriately and respectfully the nurse aide process and treatments according to the patient/caretaker’s ability to comprehend. Physical Requirements:
Ability to bend, stoop, kneel, crouch, and perform overhead lifting as needed. Ability to lift and carry items weighing more than 50 pounds
