    Your Role at a Glance

     

    As a Hospice CNA, you’ll serve as a vital part of our interdisciplinary care team, helping patients remain at home in comfort and dignity.

    Key Responsibilities:

    + Assist patients with daily personal care such as bathing, grooming, and mobility

    + Support specialized needs including catheter care, dressing changes, and ostomy maintenance

    + Monitor and document vital signs and changes in condition

    + Maintain patient dignity and privacy while adhering to HIPAA and safety standards

    + Provide emotional support to both patients and their families during a challenging time

    + Travel throughout the assigned region to provide in-home care services

    Qualifications

    + High school diploma or GED

    + Active CNA certification or completion of a state-approved Hospice Aide training program

    + At least one year of experience in hospice, home health, or direct patient care

    + Valid driver’s license, reliable transportation, and auto insurance

    + Comfortable traveling up to 50% within your designated territory
