    Pay Rate 52-54k annually

     

    Hours M-F 8-5 with flexibility required

     

    The TLC Coordinator will work closely with the MH Recovery Coordinator and demonstrate the ability to teach habilitation and recovery skills. They will understand existing service systems within the community and will be skilled in identifying appropriate resources to support recovery. In this position, an individual will engage with and develop a positive working relationship with individuals in the program, while working with the broader service team to coordinate services and positive outcomes for the individual.

    PRIMARY FUNCTIONS :

    + Perform assessments

    • Develop service plans

    • Provide restorative services

    • Be able to intervene and engage in crisis stabilization activities

    • Be responsible for timely documentation in accordance with NYSOMH and Medicaid regulations

     

    The Sr. TLC will cover a caseload of 6-8 individuals.

    ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:

    + Participate in rotating on-call responsibilities one week per month.

    + Complete documentation and reporting in alignment with agency standards.

    + Work both independently in the field and collaboratively in a hybrid team office setting.

    + Use agency vehicle or personal vehicle (mileage reimbursed) for client transportation and field visits.

    QUALIFICATIONS AND ATTRIBUTES :

    • Strong organizational and time management skills.

    • Effective problem-solving and critical thinking abilities.

    • Capacity to manage and prioritize complex caseloads.

    EDUCATION AND EXPERIENCE :

    + Experience in the provision of Mental Health Services, promoting recovery, providing crisis management, and coordinating and collaborating with community.

    + skilled counselor and leader

    + Bachelor’s degree in Human Services + Minimum 2 years’ experience in providing Mental Health services or;

    + Associates degree + minimum 5 years’ experience in human services

    PHYSICAL CHARACTERISTICS:

    + Must be capable to access all rooms in a 2-story home

    + Able to work in open space floor plan

    + Must be capable to move throughout work day

    + Occasional lifting of > _25+ pounds
