Position Summary:

The Family Partner position is grant-funded in partnership with the Boston Public Health Commission and is part of an initiative to create a network of Community Healing Response Networks (CHRN) within the City of Boston. The CHRN provides comprehensive trauma response services and community healing programming to residents of the City who are impacted by community violence. The Family Partner works in close collaboration with our Director of Community Healing Response Services, Care Navigation team and community partners within the East Boston CHRN to provide recovery services, ongoing healing modalities and support to individuals and families.

Under the direction of the Director of Community Healing Response Services, the Family Partner conducts resource assessments and collaborates with behavioral health and/or medical staff to obtain community resources for patients and families who have experienced trauma. Within developed area of expertise, provides orientation and ongoing training related to community resources to staff, medical providers, patients and families.

The Family Partner responsibilities include: outreach and engagement to those impacted by community violence and other forms of community-based trauma, case management, assistance with supports and service linkage; participation in briefing calls; conducting home visits as needed; ensuring active, accurate and consistent communication with other staff and partners; and participation in training and supervision. The Family Partner may function as a compassionate presence at community meetings, funerals, or events and conduct outreach to families and other agencies in the community affected by traumatic events.

Essential Responsibilities:

1. Determines, independently or in collaboration with other staff, the resources that will optimally meet patients' and families' needs. Develops a multifaceted service plan to meet complex patients' needs. Acts as case manager to coordinate care with medical providers. Makes independent decisions about services to recommend to a patient/family and how to teach them to become self-reliant.

2. Maintains a caseload with many patient contacts per day including phone calls, paperwork and face to face contacts.

3. Advocates on the patient's behalf in written and verbal communications. Assists patients with such tasks as connecting to community resources and other service providers.

4. Documents and advises supervisor and clinical staff of obstacles to obtaining services and potential ways to manage them. Develops liaisons with community and state agencies to facilitate negotiation of complex systems. Visits relevant agencies to broaden knowledge and relationships.

5. Makes home visits as needed and appropriate. Masters specific areas of expertise such as domestic violence, trauma-informed care, and substance-use support.

+ Provide strength based, culturally competent outreach, education, engagement, family/peer support and service linkage to clients and their families impacted by trauma. Healing services and prevention/promotion activities include support emotional accompaniment, support groups, enhancing daily living skills, coordinating services, meeting basic needs (shelter, food, healthcare, etc), developing crisis plans, offering and connecting to holistic healing modalities (e.g. Family Constellations, yoga healing arts groups, Reiki sessions, mind/body activities), and fostering empowerment and recovery.

+ Coordinate behavioral and social support services within client’s natural environment (homes, schools, community)

+ Provide response services in the community after an incident of violence that may include community canvassing, organizing coping support groups, being a compassionate presence at community meetings, psychological first aid, etc.

+ Conduct home visits as needed. Conduct outreach to community-based partners, offering resources and training around preventing and reducing the impact of violence.

+ Function as a compassionate presence at community meetings, funerals, or events and conduct outreach to families and other agencies in the community.

+ Provide regular documentation of activities.

+ Participate in trainings and supervision.

+ Conduct Program Planning, Development, and Implementation

+ Perform other tasks as required.

+ Collaborate with other health center staff, consultants, and community partners, as appropriate.

+ Provide in-service and community presentations and education to staff, parents or community organizations on trauma response and resiliency as needed.
