This vacancy is located in the SOCIAL SERVICES DEPARTMENT of Eastern Louisiana Mental Health System in Jackson, LA (East Feliciana Parish). ELMHS is the largest 24-hour, multi-site, state psychiatric complex, comprised of residential, outpatient and inpatient services.
*This posting may be used to fill vacancies in other divisions or units at Eastern Louisiana Mental Health System within 90 days of closing.
This position may be filled as either a Probational or Job Appointment (open to all qualified applicants); a promotion (open to all statewide classified and Agency wide permanent status employees); or by Detail to Special Duty. Current permanent status, classified employees may be required to accept a Probational appointed if selected. If filled as a Job Appointment, there is a possibility that this position may be converted to a Probational Appointment should funding become available.
Minimum Qualifications
Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Certified Social Worker (CSW) plus:
A master's degree in social work.
NOTE:
Permanent status will not be granted in this job title without LMSW or LCSW licensure in Louisiana; Certified Social Worker (CSW) credential will not be accepted for granting permanent status.

Job Duties and Other Information
Job Duties:
The incumbent of this position serves a population of early, middle, and late adulthood and receives direct supervision from a Social Worker 5-B.
35% Social Work Duties and Functions/Discharge Planning: Develops in cooperation with the patient an appropriate discharge and aftercare plan. Documents the discharge plan on the social assessments and the treatment plan and updates it as needed. Locates suitable Permanent Supportive Housing, placements in nursing, group, or licensed boarding homes for those patients unable to return home or require placement. Provide agencies with required information for referrals. Schedules all aftercare appointments at local MHCs, SACs, private providers, and/or agencies as needed for continuity of care. Coordinates with regional SPOE's, or the SMO, for the smooth transition of care from the hospital setting to the community or the next level of service delivery. Establishes and maintains effective professional relationships with other agencies and professional care givers. Complete all DHH/ OBH/ COC aftercare referral documentation for chart and agencies' needs. Document all aftercare appointments and other referrals on discharge forms, progress notes and Continuity of Care/ OMHIIS database.
20% Social Work Duties Functions/ Clinical Interventions and Progress Documentation: Conducts goals directed and supportive individual and group treatment with affected adult patients. Conducts family treatment with patient's significant others as needed. Implements appropriate treatment interventions and explain the treatment plan to the patient. Encourages patient and significant others to actively participate in treatment through didactic discourse and appropriate professional feedback. Document Active treatment summaries on each patient chart specifying the patient's progress towards treatment objectives and goals, and response to interventions and reason for continued stay using the BIRP documentation method. Documents daily, for billing, all social services provided. Documents daily pertinent information needed by other treatment team providers for optimal care.
20% Social Work Duties and Functions/Psychosocial Assessment: Collects and integrates social assessment information from patient, family, and other available resources into a complete social assessment within agency delineated timelines. Provides for ongoing monitoring of patients' commitment status.
20% Social Work Duties and Functions/ Treatment Planning: Participates in the development of the Master Treatment Plan Position Description SCS will keep this document for six (6) years. Page 3 of 5 using S.M.A.R.T. method. Presents social assessment to the multi-disciplinary team to be used in formulating the problems, treatment goals and corresponding patient objectives, or lack of, and record same on the Master Treatment Plan and update forms found in the chart. Documents on treatment plan method the presenting problem, treatment objectives, the treatment interventions the patient will receive from social services staff, the discharge plan and signature. Complete the Patient Information Program (PIP) forms corresponding with the treatment plan activity. Give ongoing verbal feedback to the treatment team on the patients' progress each day during morning rounds. Maintain high degree of professionalism with all staff disciplines and members of the treatment team to promote team cohesion and effectiveness.
5% Participate in educational trainings that are mandatory for accreditation and licensure of the facility, including but not limited to, CPR, CPI, and First Aid. Perform all other duties assigned by supervisor/designee to maintain continuity and quality, accreditation and licensure requirements. Perform all other duties assigned by supervisor, but not listed, for the smooth operation of the unit/building and/or the facility.
Maintains current, valid, and unencumbered motor vehicle operator’s license.
Adheres to all rules, regulations, policies and procedures of LDH, OBH, Eastern Louisiana Mental Health System and Civil Services.

SPECIAL ENTRANCE RATE (SER) OF PAY: Social Worker 1-$1,900.00-bi-weekly / $23.75-hourly
Premium Pay: From $2.00 to $5.00 per hour for facility longevity pay
