Pay Range: $22.50 - $25.50/hour, depending on applicable experience
Job Summary:
The Navigator supports individuals who have been deemed eligible for enhanced services in the Social Care Navigation (1115 Waiver) program for Housing, food, and/or Transportation. This position involves case management, advocacy, and ongoing support to ensure individuals receive the care they need. It includes housing services such as Pre-tenancy, housing transition, and/or housing sustaining; pantry stocking through the food pantry at OLV Human Services, and others. The Navigator communicates with Landlords, utility companies and other community resources to ensure the individuals are getting the assistance they need. The Navigator initiates payments for housing based on a fee schedule, or communicates with the OLVHS food pantry for pantry stocking boxes to eligible individuals.
Essential Job Duties:
+ Serve as the primary point of contact for individuals with social health needs in housing, food, and/or transportation ensuring consistent communication.
+ Coordinate and schedule services, appointments, and follow-ups for participants.
+ Document progress and action taken with each referral and every interaction in the WNY Integrated Care IT platform, as required.
+ Initiate payments through WNYICC Social Care Platform and OLV HS according to fee schedule and eligibility.
+ Conduct In-Home Assessments with community members as needed.
+ Handle customer questions and concerns with excellent customer service, empathy, and patience.
+ Attend WNY Integrated Housing/Nutrition/Transportation Workgroup.
+ Participate in staff meetings, training, and quality improvement initiatives.
+ Demonstrates excellent customer service on phone, in-person, via email and in all aspects.
+ Educate participants on available program benefits and rights under the 1115 Waiver/ Social Care Navigation.
+ This position is responsible for working within WNY Integrated Care social care technology and transportation platforms.
+ Advocate for participants to ensure they receive timely and equitable access to care and services.
+ Monitor participant progress and outcomes
+ Collaborate with service providers, agencies, and other stakeholders to create a seamless care experience.
+ Maintain accurate and detailed documentation of all participant interactions and service coordination efforts in compliance with program policies and HIPAA regulations.
+ Participate in staff meetings, training, and quality improvement initiatives.
+ Any other duties assigned by Supervisor
Skills:
+ Strong organizational, problem-solving, and advocacy skills.
+ Ability to work independently and manage multiple tasks efficiently.
+ Knowledge of community resources and 1115 Waiver program guidelines.
+ Proficiency in Microsoft Office and electronic health record (EHR) systems.
+ Excellent interpersonal and communication skills, with the ability to work with diverse populations.
Minimum Education Required:
+ Bachelor’s degree in social work, public health, human services, or a related field preferred.
Minimum Experience Required:
+ Experience in case management, care coordination, or a similar role within social services or healthcare.
+ Previous experience in eligibility screening, social services, or healthcare preferred.
License/Registration/ Certifications Required: NYS Driver’s License
Training Requirements:
+ New Hire Orientation
+ All 1115 Screener and Navigation trainings by WNYICC
Physical Requirements:
The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required. There is little, if any, exposure to environmental conditions.
• The constant physical demand of the position is sitting.
• The frequent physical demands of the position are standing, walking, climbing, talking, hearing, repetitive motions, and eye/hand/foot coordination.
• The occasional physical demands of the position are lifting, carrying, pushing, pulling, balancing, stooping, crouching, reaching, handling and grasping
