The Care Center RN is responsible for providing assessment services to patients and their families who are presented for potential hospitalization for treatment of psychiatric or emotional disorders; to support the hospital and program philosophy of care; to ensure the safety and well-being of each patient. The Care Center RN will provide prompt response to incoming calls for assessments and review of clinical referrals for admission.
Essential Duties:
+ Documents Care Center calls in accordance with Care Center policies
+ Conducts reviews of referral packets in a timely manner and in accordance to hospital policy
+ Ensures administrative approval obtained for all admissions
+ Completes admission data entry paperwork in accordance with hospital policy
+ Ensures follow up calls are made to those failing to show up for assessment appointments
+ Pre-certification is obtained in accordance with insurance requirements, and verification of benefits email is sent
+ Promptly notifies department leadership/AOC of problems or significant events
+ Safety problems are identified and corrective actions taken
+ Answer alarms and investigate disturbances.
+ Responds to and documents telephone and face-to-face inquiries from persons seeking mental health care or information about mental health-related services.
+ Ensures all “secret shopper” calls are successful, using the hospital-approved scripting.
+ Complete admissions paperwork (as applicable)
+ Plans and provides nursing care for patients through a systematic process of assessment with patients, the patient's significant others, and other appropriate health care providers
+ Collects and analyzes data regarding the patient's physical, psychosocial, environmental, self-care, educational, spiritual, cultural, and anticipated discharge needs.
+ Ensures each patient's unique needs are identified and incorporated into a speedy acceptance (or denial) of request for admission that includes the severity of diseases, conditions, and impairments, particularly as it relates to established or emerging suicide risk.
+ Collaborate with interdisciplinary professionals to determine and provide the best possible care for patients.
+ Provides social support for patients by creating and encouraging patients to take advantage of opportunities such as inpatient or outpatient admission, support groups, social activities and other therapeutic measures.
+ Conducts assessments through interviews with patients and relatives to gather case history.
+ Communicates assessment data to the psychiatrist for purposes of establishing inpatient or outpatient admissions.
+ Promptly communicates acceptance/denial of admission to referral sources.
+ Notifies on-call physicians for necessary assessments & admitting orders.
+ Hands-off a cogent report to the Intake RN for purposes of receiving the patient for admission & continued assessment.
Requirements
Job Requirements
Must be 21 years old or older
Education - ADN or BSN from an accredited college or university school of nursing.
Experience – Minimum of 2 years clinical experience, preferably in a behavioral healthcare setting, phone triage, tele-health or case management or combination of education and experience. Basic teaching and training skills a plus. Required Licenses - Valid Texas RN license in good standing required.
Additional Requirements: Must pass a medication administration exam before administering medications. Must obtain and maintain approved CPR certification as well as successful completion of Handle with Care training including restraint and seclusion policies prior to assuming patient care responsibility.
