    Colorado Veterans Community Living Center (VCLC) at Fitzsimons is a beautiful 180-bed, skilled nursing facility serving Colorado’s veterans.     Description of Job

    This position is in training to provide basic hands-on patient/resident care in a safe, clean, infection controlled environment by bathing, dressing, grooming, hygiene, skin care, range of motion, positioning, ambulating, transferring, toileting programs, feeding programs, transporting, performing basic assessments (vital signs, blood pressures, weights, etc.), and charting/documenting all of the above.

    Job duties include, but are not limited to:

    + Under the direction of nursing staff in training provide direct resident care to include bathing, dressing, undressing, assisting in and out of bed, performing personal hygiene, feeding programs, toileting programs, range of motion, transporting, and interacting socially to meet residents’ basic needs. Explaining patient care before, during and after contacts; answering call lights; and assessing basic resident care needs; and updating noted resident changes

    + Under the direction of nursing staff perform basic assessments, vital signs, blood pressure, weight, intake and output, basic skin assessments

    + Under the direction of nursing staff provide infection control, implementing/using universal precautions, and disposal of waste. Clean linen room and equipment, stock supplies, complete supply forms

    + Under the direction of nursing staff performs correct use of safety/positioning devices, side rails, transfer belts, proper use of equipment, attend on-going staff education programs.

    Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

    Special Qualifications:

    Must hold and maintain an active CNA certification in good standing from the Colorado Department of Regulatory Agencies, as well as a current BLS Healthcare Provider Card.

    Minimum Qualifications :

    + High school diploma or General Educational Diploma (GED).

    Substitutions:

    + Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED.

    Preferred Qualification :

    + Experience in a long-term care/assisted living environment

    + Experience transporting/driving residents

    + Excellent communication and customer service skills

    + Relevant State of Colorado experience

    Conditions of Employment:

    + Full Background CDHS employees (all Direct contact with vulnerable persons):

     

    CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, PREA (Division of Youth Services), Trails check (direct contact with children), CAPS (direct contact with adults – Mental Health Institutes, Regional Centers, Veterans Community Living Centers)

     

    + TB Test/Flu Vaccine

    + Must be willing to obtain vaccinations, as required by the State and facility

    + Must possess a valid, unrestricted U.S. driver's license

    + May be required to travel/transport residents in state vehicles

    + Required to have a current, valid BLS Healthcare Provider Card

    + Shift Work: While every effort will be made for the schedule to reflect the shift/days agreed upon by my supervisor and me, I understand I may be scheduled on different days/shifts based on facility need. While leadership will ask for overtime volunteers as a first-line strategy to relieve staffing shortages, I understand that I may be required to work overtime as a part of this position. Shift work includes weekends, nights, holidays

    + May require on-call status, if needed, per the pay plan for this position

    + Former State employees who were disciplinarily terminated or resigned in lieu of termination must (1) disclose that information on the application (2) provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position, (3) provide the employee number from the applicant’s prior State employment. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination and failure to provide this information will disqualify the applicant from future State employment with CDHS.
