Job Description:

JOB SUMMARY

The Activities Coordinator Senior Services will plan activities for all residents based on the comprehensive assessment and care plan and the preferences of each resident. There will be an ongoing program that supports residents in their choice of activities, both facility-sponsored group and individual activities and independent activities, designed to meet the interests of and support the physical, mental, and psychosocial well-being of each resident, encouraging both independence and interaction in the community.

JOB QUALIFICATIONS

EDUCATION/EXPERIENCE

_For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation._

Minimum Required:

Qualified therapeutic recreation specialist or an activities professional who has one of the following:

(A) Eligible for certification as a therapeutic recreation specialist or as an activities professional by a recognized accrediting body on or after October 1, 1990; or

(B) Has 2 years of experience in a social or recreational program within the last 5 years, one of which was full-time in a therapeutic activities program; or

(C) Is a qualified and licensed occupational therapist or occupational therapy assistant awarded by state of WI; or

(D) Has completed a training course approved by the State.

Preferred/Optional:

Bachelor’s degree in therapeutic recreation

CERTIFICATIONS/LICENSES

_The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position._

**Minimum Required:** Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. Certified Nursing Assistant Course completion within six months of hire. Occupational Therapist or Certified Occupational Therapy Assistant (COTA) license in the state of Wisconsin if meeting Education/Experience criteria “C” above.
