**Shift** : Day

**Location:** Lake County and Winter Haven

The role you’ll contribute:

Provides patient care in their home, following physician's orders.

The value you’ll bring to the team:

+ Initial assessment of patient in their home.

+ Assures physician orders for care meet regulatory requirements.

+ Ongoing evaluation of client in a holistic manner.

+ Plans patient care in consultation with Case Coordinator Nurse.

+ Supervision of LPNs and Home Health Aides and updates HHA Care Plans.

+ Provide accurate, current documentation.

+ Schedules daily visits to facilitate patient needs.

+ Demonstrates good time management skills.

+ Maintains proficiency in clinical and assessment skills.

+ Participates in Patient Case Conferences.

The expertise and experience you’ll need to succeed** **:

Minimum qualifications:

+ 1 year experience in medical surgical setting

+ Current nursing license in the State of Florida

+ Valid in state Driver’s License with current car insurance

+ Active American Heart Association BLS

Preferred qualifications:

+ Home Care experience
