Job Summary
The Community Outreach Coordinator is responsible for planning and implementing evidence-based health promotion programs and services designed to improve the health and well-being of students, athletes, families, and communities. This role involves collaborating with school administrators, community members, and business leaders to build partnerships, coordinate events, and increase the community's knowledge of healthcare services.
Essential Functions
+ Plans, implements, and coordinates health promotion initiatives in schools, communities, and businesses, focusing on improving health outcomes and increasing awareness of healthcare services.
+ Collaborates with key stakeholders, including school administrators, community leaders, and hospital staff, to develop comprehensive, multidisciplinary health programs that address the specific needs of the community.
+ Coordinates and schedules health education programs, training sessions, and job shadow opportunities for students, parents, educators, and community members.
+ Recruits volunteers to assist with the delivery of health programs and community events.
+ Serves as a liaison between schools, businesses, and healthcare providers, promoting new programs and initiatives that improve health outcomes based on evidence-based practices.
+ Manages outreach programs such as Community Link and Patient & Family Advisory Council, coordinating meetings, events, and maintaining up-to-date materials in collaboration with the marketing team.
+ Acts as a resource for patients, helping them navigate healthcare services and connect with appropriate resources to address healthcare barriers.
+ Prepares and distributes health education materials, including brochures, posters, reports, and visual aids, for use in schools, community events, and outreach programs.
+ Coordinates with the marketing team to develop press releases, signage, and media campaigns to promote school and community collaborations.
+ Collects data to evaluate program effectiveness, prepare reports, and make recommendations for improvements.
+ Provides support to patients, families, and community members, ensuring a clean, safe environment and assisting in emergencies, including CPR and AED use when needed.
+ Performs other duties as assigned.
+ Complies with all policies and standards.
Qualifications
+ 1-3 years of experience in community outreach, health promotion, or a related field, with experience coordinating events and working with schools and community organizations required
Knowledge, Skills and Abilities
+ Strong organizational and multitasking abilities, with the ability to manage multiple projects simultaneously.
+ Excellent communication and interpersonal skills for building and maintaining relationships with community partners, volunteers, and healthcare professionals.
+ Knowledge of evidence-based health promotion strategies and best practices in public health.
+ Proficiency in using Microsoft Office Suite and other relevant software to manage data, create reports, and develop educational materials.
+ Ability to work independently and as part of a team, demonstrating initiative and leadership in community outreach efforts.
