    Hospice RN Case Manager

     

    $10,000 Sign-On Bonus + Day-One Benefits

    Service Area: Dubuque, Jackson, Delaware, Jones, or Clayton Counties

    • Your Role: Hospice RN Case Manager

     

    As a **Hospice RN Case Manager** , you’ll provide one-on-one care to patients with life-limiting illnesses, helping them live with dignity, comfort, and peace. You’ll lead the care team, coordinate services, and support patients and families through every step of their journey.

     

    • What You’ll Do

    + Develop and manage individualized care plans

    + Provide symptom management and emotional support

    + Educate patients and families on end-of-life care

    + Coordinate with physicians and interdisciplinary teams

    + Supervise Hospice Aides and LPNs

    + Document care accurately using point-of-care technology

    + Participate in IDT/IDG meetings and case conferences

    + Ensure timely communication and care coordination

    • What You’ll Need

    + Graduate of an accredited nursing program

    + Active RN license in the State of Iowa

    + Minimum 1 year of professional nursing experience (acute care preferred)

    + Hospice or home care experience preferred

    + Strong communication and interpersonal skills

    + Valid driver’s license and reliable transportation
