This position will carry out physical therapy treatments and procedures, addressing areas of prevention, maintenance and restoration, and assist in the development and provision of restorative and maintenance programs to maximize resident abilities in accordance with established policies and procedures of the facility as well as the individualized care plans for each resident.

Minimum Qualifications

+ Current license as a Physical Therapy Assistant in Minnesota with no restrictions.
Preferred Qualifications
+ Any combination of 2 years or more experience working as a physical therapist assistant in the following settings: long term care, skilled nursing, or neurological rehab (either outpatient or inpatient).
Additional Requirements
This position also requires successful completion of the following:
A Minnesota Department of Human Services (DHS) background check will be conducted on finalists for this position. Finalists must pass the DHS background check.
A Tuberculosis (Mantoux) screening (or chest x-ray) is required prior to employment.
Employment reference checks will be conducted on finalists. This may include a review of documentation related to job performance and contact with the applicant's former employer(s).
