    Naval Hospital Guam is a 282,000 square foot facility and is comprised of the main hospital in Agana Heights, and a branch health clinic, at Apra Harbor on Naval Base Guam. The Naval Hospital consists of nearly 700 staff who serve more than 27,000 beneficiaries.

    Responsibilities

    This position is located in a hospital or clinic setting and its purpose is to dispense drugs prescribed by physicians and other health practitioners and to provide information to health practitioners and patients about proper usage of medications.

    MAJOR DUTIES AND RESPONSIBILITIES

    1. Clinical Specialty Activities.

    a. Dispenses, prescribes and/ or administers medicine. Responsible for evaluation, transcription and interpretation of a physician’s prescription order (written and verbal) for appropriateness of drug therapy to include evaluation of the drug selected and the regimen prescribed (dose, frequency, duration). Interviews patients to assess for drug allergies and past adverse medication reactions. Screens for drug interactions and reviews pertinent laboratory results, when necessary, as part of the prescription verification process. Conducts medication reconciliation, reviews the prescription profile for redundant therapies, and discontinues medications with remaining refills when appropriate. Documents pharmaceutical care activities in the medical record as appropriate. Prepares, compounds, and dispenses pharmaceuticals for patients of all ages, conduct education for patients and healthcare professionals, and ensure proper handling and dispensing of investigational drugs. Prescribes medications as authorized per command-approved protocols. Oversees technician staff that process and fill prescriptions and/or sterile and non-sterile pharmaceuticals.

    b. Monitors drug therapy to assure clinically appropriate balance of risks and benefits is maintained and modifies the therapy when it differs from established clinical parameters or protocols. Interfaces with physicians or other health care providers in the design of a drug treatment plan and monitors drug therapy for patients. Responsible for the monitoring, detecting, documenting, reporting, and managing of adverse drug events. Documents relevant findings from drug therapy monitoring activities or other pharmacist intervention activities to ensure interventions are communicated to other health care professionals who are responsible for the patient's care in order to increase the effectiveness and minimize potential risks of drug therapy.

    c. Prepares complex combinations regimens for clinical protocols including new and investigational agents and biologic response modifiers. Designs drug regimens and orders changes in therapy and appropriate laboratory tests after patient monitoring and drug therapy review. Participates in daily rounds to monitor and review drug-therapy for rational indications, dosing, adverse drug reactions, IV incompatibilities and complications of multiple drug therapies.

    d. Evaluates drug literature. Compares and contrasts therapeutic regimens and use for new and existing products. Provides drug information to physicians, nurses, and patients on investigational drugs being utilized and ensures that up to date information is readily available in the patient care units. Responsible for participation in drug use evaluations and other quality assurance activities.

    2. Clinical Teaching Activities. Designs and conducts interdisciplinary educational programs in the areas of drug therapy for physicians, nurses and other health professionals. Using suitable oral, written, or audiovisual communication techniques and methods, develops or assists other health care practitioners in the development of educational materials and informs, educates, and counsels patients about their drug therapy in order to maximize proper use of medications to improve therapeutic outcomes.

    3. Other administrative duties as assigned.

    Qualifications

    Who May Apply: US Citizens

    **Basic Requirements** for Pharmacist:

    **Education:** A doctoral degree in Pharmacy that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

    **Licensure:** A current, active, valid, and unrestricted license to practice pharmacy in a State, a territory of the United States, or the District of Columbia.

    **Medical Requirement:** Must be able to distinguish basic colors.

    **Experience:** One year of professional pharmacy experience which includes **preparing pharmaceuticals, providing medication counseling to patients, and providing pharmaceutical education to healthcare professionals** .

    Education

    **FOREIGN EDUCATION:** If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.     **Basic Life Support (BLS)** : Required, BLS training certification from, or in accordance with, the American Heart Association and American Red Cross. Current Advanced Life Support or other advanced certification does not supersede Basic Life Support completion.
