    Salary:$80,000 annually

     

    Hours- Tue to Sat 11 am to 7 pm

     

    $5,000 Pay to Stay Bonus

    LMSW/LMHC/LMFT/LCSW

    Overview

    Under the guidance of the Director of the Safe Options Support (SOS) Critical Time Intervention (CTI) Team, the Community Outreach Clinician plays a vital role in supporting individuals experiencing homelessness as they transition toward safe and stable housing. This position combines outreach, supportive counseling, advocacy, and care coordination to help people access the services they need in a compassionate and respectful way.

    What You’ll Be Doing

    As part of a collaborative, interdisciplinary team, the Community Outreach Clinician will engage individuals who are unhoused through outreach in the community. You will help build relationships and trust with people in unsheltered living situations—such as encampments, public spaces, and emergency settings—using a person-centered, trauma-informed approach.

    Your work will involve:

    + Assisting individuals in accessing shelter and permanent housing

    + Supporting the development of daily living skills and independence

    + Providing accompaniment to appointments and services

    + Advocating for equitable access to healthcare and other supports

    + Helping build ongoing connections to trusted community resources

     

    You’ll follow participants as they transition into housing, ensuring continued connection, stability, and success in their new living environments. Member choice, harm reduction, flexibility, and dignity-driven care are core values of the SOS program.

    Key Responsibilities

    + Conduct community-based outreach to identify and engage individuals in need

    + Assess participant strengths, needs, and goals through conversation and observation

    + Utilize evidence-based approaches such as Motivational Interviewing and Trauma-Informed Care

    + Support individuals with housing applications, interviews, and move-in preparation

    + Participate in hospital discharge planning to ensure smooth transitions

    + Collaborate with community providers and service organizations

    + Assist with transportation and appointment navigation

    + Facilitate access to basic needs and supportive services

    + Provide crisis support and emotional assistance as needed

    + Ensure timely and accurate documentation of services in electronic health records

     

    What We’re Looking For

    Minimum Qualifications:

    + Licensed Master’s degree (LMSW, LMHC, LMFT, LCSW, etc.), or RN with experience in behavioral health and/or substance use support

    + Passion for helping individuals experiencing homelessness or housing instability

    + Familiarity with Orange and/or Dutchess County resources is a plus

    + Comfort working in community-based, field-oriented settings

    + Strong interpersonal skills: approachable, respectful, collaborative

    + Drivers license

    Work Environment & Schedule

    This is a community-based role that involves fieldwork throughout Orange and Dutchess Counties. The position requires schedule flexibility to meet program and participant needs. Team support and scheduling accommodations are in place to ensure work-life balance.
