    Palliative Care Social Work Fellow Opportunity- Danbury, CT

    Summary:

    The Palliative Care Social Work Fellow works under the supervision of the Nuvance Health Palliative Care Social Work Fellowship Program Supervisor to maintain and enhance a comprehensive program of services for patients and their families and aids in the adjustment to living with chronic and life-threatening illnesses in collaboration with the interdisciplinary treatment team. The scope of the position includes clinical practice of counseling/therapy, program evaluation, and participation in educational/training activities, in-service activities, research, and advocacy. Provides psychosocial support to palliative care patients through the palliative care services consult team. Applies knowledge of psychosocial care in support of hospital quality improvement initiatives.

     

    This is a one-year paid fellowship, benefits eligible.

    Responsibilities:

    1. Under supervision, collaboratively, and independently provides social work interventions to meet the bio-psychosocial needs of patients with and their families or significant others throughout the disease and treatment continuum in the form of both supportive/illness adjustment counseling and resources/concrete service assistance to maximize patient/family/caregiver illness adjustment and minimize related psycho-social distress. This includes assisting with coordination of care and complex social situations between the inpatient setting and ambulatory care clinic.

    2. Performs/documents psychosocial assessments and develops and implements an individualized treatment plan that enhances the overall functioning and coping of the patient and his/her family.

    3. Assists the palliative care team and/or directly facilitates patient-family meetings for goal setting and care planning.

    4. Works in a multidisciplinary, collaborative team effort with physicians and other healthcare team members in determining patient's medical status, treatment plan and prognosis as they relate to psychosocial care, long-term care, and end of life care planning. Makes necessary referrals in collaboration with other interdisciplinary team members as needed, with appropriate follow-up.

    5. Participates in discharge planning process.

    6. Participates in educational and outreach services relevant to the social service; palliative care interface.

    7. Participates in palliative care quality improvement initiatives.

    8. Prepares correspondence and maintains accurate and legible case records in the appropriate manner that outline the monitoring, evaluating, and recording of client progress according to measurable goals.

    9. Provides liaison function between palliative care team and hospital quality improvement, discharge planning, and social service departments.

    10. Provides liaison function between palliative care team and community hospice, home care agencies, long- term care facilities and other community services as needed.

    11. Attends monthly and periodic team meetings for the operational and administrative functions of the palliative care service.

    12. Contributes to supportive interventions for staff to reduce work related stressors and offer psychosocial support. Also provides education for staff.

    13. Attends and actively participates in daily interdisciplinary team rounds and contributes to interdisciplinary education programs including didactics, debriefings, and case review.

    14. Adheres to the Professional Social Work Code of Ethics as set forth by the National Association for Social Workers (NASW).

    15. Complies with all patient confidentiality policies and government regulations.

    16. Performs other responsibilities or duties as assigned by the Director of Palliative Care.

    17. Fulfills all compliance responsibilities related to the position.

    18. Maintains and Models Nuvance Health Values.

    19. Demonstrates regular, reliable and predictable attendance.

    20. Performs other duties as required.

    Other Information:

    The Fellowship is a one year position

    Manual: Some manual skills/motor coord & finger dexterity

    Occupational: Some occupational risk

    Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force
