    Organ Referral Coordinator

     

    Full Time, Exempt

     

    $60,405.80 annually

    Schedule:

    Variable 8hr and 12-hr shifts

     

    Rotating Weekends and holidays

    On-call required

    Local Travel Required

    Job Summary:

    Responsible for the evaluation of imminent death referrals to include decision-making regarding potential donation opportunities. In addition, should engage physicians, practitioners, ConnectLife Medical Director, and ConnectLife Administrator-On-Call as resources for evaluation. Provide continuous education to administrative and clinical hospital staff regarding the referral process.

    Essential Job Functions:

    + Collects information to evaluate medical suitability of an organ donation referral utilizing information from medical records, history and physical examination, and current health status.

    + Discusses all referrals with Administrator-On-Call to determine suitability.

    + Take on-call as a member of the Organ Services team. Completes effective daily rounds at high potential hospitals as well as rounds on open imminent death referrals.

    + Collaborates with physicians and other hospital staff to develop plan for referral management from initial referral until initial approach. Activate family services staff and clinical donation coordinator staff and provide report when appropriate.

    + Documents complete, accurate, and timely information of referral and donor related activities in the confidential donor record, forms, and database according to established policies and procedures.

    + Ensure compliance with UNOS, CMS and ConnectLife standards for potential organ donor evaluation.

    + Provide follow-up on new information related to imminent death referrals including lab work, radiology studies, etc.

    + Provide feedback to the referring hospital as necessary.

    + Participate in Hospital Development activity as necessary.

    + Attends and participates in departmental meetings either in person or by virtual means.

    + Make a proactive effort to stay informed and up to date regarding changes in the field.

    + Maintain/obtain any necessary certification and/or education required for the position.

    Education and Experience:

    + Associates degree or certification in a related health care field required.

    + Bachelor’s degree in a related field preferred.

    + At least one year experience in a clinical setting to include an understanding of patient hemodynamics and treatment modalities for hemodynamic instabilities; OR

    + Clinical or hospital experience in critical care, urgent care, emergency room; OR

    + At least one year of experience in a related health care position

    Knowledge, Skills, and Abilities:

    + Excellent interpersonal skills, written and verbal communication skills required, including communicating with diverse individuals and public speaking.

    + Must be adaptable and flexible to change with the ability to think critically in solving problems with a continuous focus on customer service.

    + Strong and effective relationship building skills required to work in partnership with various levels of hospital staff.

    + Proficiency in Microsoft Office and data entry required.

    + Must be detail-oriented and remain accurate while under pressure.

    + Must be results oriented, and highly organized with the ability to think logically, manage time effectively, prioritize and multi-task.

    + Must be able to remain professional in making decisions and handling confidential information.

    + Must have patience and maintain focus through interruptions and maintain emotional control under stress.

    + Experience working in a collaborative team environment strongly preferred.

    + Must be self-motivated, reliable and have a passion for donation.

    + Must have professional appearance and demeanor.

    + Must have a valid NYS Drivers license and reliable transportation. Local travel required

    + On-call required.
