• PrEP Case Manager

    Job Summary:

    The primary responsibility of this position is to provide supportive services to patients enrolled in the SUNY Upstate PrEP Program. The PrEP Program Case Manager is an important member of the patient services team and works closely with the team to provide excellent patient centered care. The PrEP Program Case Manager works with patients from the point of referral to the program to the point of discharge. The Case Manager will also assist with case finding and support outreach and program promotion.

    Minimum Qualifications:

    Bachelor's Degree in Public Health, Social Work, Mental Health, Human Services or related degree or a combination of education and hands on experience in a related discipline.

     

    *Candidates with an Associates Degree and 2 years of related experience will be considered.

    Preferred Qualifications:

    Previous case management experience preferred. Previous knowledge of HIV prevention and health care preferred.

    Work Days:

    Monday-Friday, 8:00 am-4:30 pm.
