    Job Description

    _Outpatient Certified Advanced Nutrition Coordinator-Mount Sinai Hospital_

     

    Under the direction of Clinical Nutrition leadership (Senior Director, Director, Clinical Nutrition Manager), the Outpatient Certified Advanced Nutrition Coordinator (CANC) is responsible for activities that support patient focused care within their clinical specialty area. The CANC is an advanced and certified expert in his/her specialty and leads nutrition services in their designated ambulatory practice. The CANC coordinates activities for the practice, identifies performance opportunities, and supports the goals of the program. The CANC adheres to department/hospital philosophies, standards, objectives, and policies and procedures.

    Qualifications

    _Requirements:_

    + Bachelor’s degree or higher from an accredited institution, with completion of ACEND (Accreditation Council for Education in Nutrition & Dietetics)-accredited dietetic internship.

    + 5+ years of clinical experience, including 2-3 years in specialty area Research experience preferred Registered dietitian (RD) from the Commission on Dietetic Registration (CDR) required Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions required or process initiated upon hire.

    + Advanced Certification, i.e. Certified Diabetes Educator (CDE), Certified Nutrition Support Clinician (CNSC), and/or Pediatric/Renal/Oncology Specialty required

     

    Compensation Statement

     

    The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $72,473.00 - $108,709.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.

     

    Non-Bargaining Unit, 209 - FS Clinical Nutrition - MSH, Mount Sinai Hospital

    Responsibilities

    + Demonstrates expertise in a clinical focus area with advanced credential. Exhibits innovative thinking in developing care plans for patients within practice.

    + Evaluates the nutritional status of the patients by reviewing pertinent information obtained from the electronic medical record and the interdisciplinary healthcare team. Applies advanced practice experience, credential, and knowledge of nutrition screening and assessment to coordinate and provide medical nutrition therapy by developing and implementing individualized nutrition care plans for general and practice specific patient populations.

    + Provides individualized counseling to patients and caregivers, including management of food-drug interactions, therapeutic diets, and supplements.

    + Conducts nutrition-focused physical exams, as indicated, to determine risk and/or degree of malnutrition.

    + Documents all encounters in the electronic health record per Clinical Nutrition policy.

    + Completes necessary paperwork to support insurance billing/reimbursement process.

    + Completes and maintains administrative records, including productivity, annual competencies, and other documentation of direct and non-direct patient care functions, with timely submission to direct supervisor.

    + Functions as a coordinating member of the multidisciplinary team leading discussions regarding the nutrition care of patients in the practice. Participates in interdisciplinary rounds and care plan meetings as needed.

    + Organizes projects and initiatives for the ambulatory practice that support program development, including in-services, education programs, lectures, community-focused programs and external speaking events.

    + Evaluates patients on nutrition support and communicates with the clinical care team. Coordinates team meetings and discussions related to nutrition support provision. Assists with discharge planning and hospital intake as needed.

    + Demonstrates leadership through the coordination and implementation of initiatives that promote nutrition services in the practice.

    + Engages in activities that support professional growth and development of self and nutrition colleagues in the practice, further advancing nutrition expertise in focus area.

    + Organizes quality assurance monitoring and reporting activities. Engages in performance improvement projects that support goals of ambulatory practice.

    + Designs, develops, and regularly updates nutrition education materials to meet the specialized needs of the target patient population.

    + Precepts dietetic interns and instructs them on the competencies required to complete their assigned rotation, develops learning modules and completes student evaluations.

    + Coordinates research activities for ambulatory practice.

    + Maintains a positive and professional demeanor and effectively communicates with patients, visitors, colleagues, hospital staff, and supervisors.

    + Accepts and incorporates feedback for clinical development.

    + Supports department/program goals. Adheres to department/hospital policies and procedures, quality standards and safety standards.

    + Performs other duties as assigned.
