    Job Title: Licensed Practical Nurse Care Coordinator

    Job Description

    We are seeking a dedicated Licensed Practical Nurse (LPNr to join our team. In this role, you will prepare patients for visits by assessing their chief complaints, reconciling medication lists and allergies, and evaluating smoking status. You will ensure exam rooms are ready and maintain medical equipment. This position requires meticulous documentation in electronic medical records and a proactive approach to obtaining necessary medical records for upcoming appointments.

    Responsibilities

    + Prepare patients for visits by assessing chief complaints and reconciling medication lists and allergies.

    + Prepare and restock exam rooms, sterilize instruments, and maintain medical equipment.

    + Document interventions in electronic medical records and review charts for completeness.

    + Scan upcoming appointments for additional medical record information and obtain records from other healthcare providers.

    + Respond to patient messages, prescription refills, lab results, referrals, and follow-ups from physicians.

    + Collect patient satisfaction surveys and contact patients to assess medical status and medication adherence.

    + Follow protocols related to infection control, environmental safety, and patient confidentiality.

    + Complete annual recertification for OSHA, Compliance, Respiratory FIT, and IT policy.

    Essential Skills

    + Ambulatory care experience

    + Outpatient care expertise

    + Licensed Practical Nurse (LPN)

    + Familiarity with geriatrics and memory care

    + Proficiency in using electronic health records, specifically Epic

    + Experience with triage and managed care

    Additional Skills & Qualifications

    + Practical Nurse License

    + Ambulatory care expertise

    + BLS certification

    Work Environment

    This role is based in an outpatient office with working hours from Monday to Friday, 7:30 AM to 5:00 PM, including an hour-long lunch break. There are no weekend or holiday shifts. The environment is conducive to learning, offering opportunities for permanent positions and career growth within the healthcare industry

    Pay and Benefits

    The pay range for this position is $27.00 - $30.00/hr.
