This position will serve as the Nutrition Education Navigator for Cornell Cooperative Extension Association of Jefferson County for the 1115 Medicaid Waiver program, and as such will provide screening, navigate referrals, and implement the nutrition portion of the 1115 Medicaid for both adults and children. This position will facilitate Medicaid 1115 waiver activities to include phone calls, home visits, virtual visits, and community educational activities. These activities will screen Medicaid members for eligibility and referral needs. The educator will ensure participants follow through with referrals. Finally, the position will provide direct nutrition education to eligible referrals using evidence and research-based curricula. This position will also coordinate all administrative aspects of the Medicaid 1115 Waiver program, which includes securing new partnerships to gain access to referral sources, coordinating efforts with other staff, working with Healthy Alliance, working with CCE’s Finance Department with billing and reporting aspects. This position will also assist the Nutrition Program Manager with marketing efforts to promote the new program.

POSITION DETAILS:
This position is a full-time non-exempt position, 40 hours per week. The hourly rate for this position is $24.25. This position, based on eligibility, offers a comprehensive benefit package, to include: health and dental insurance, NYS retirement, vacation, sick and personal leave.
POSITION INFORMATION:
Required Qualifications:
+ Master’s Degree or Equivalent Education (Bachelor’s Degree and four (4) years of transferable relevant evidence-based research experience) with training and experience to provide nutrition education.
+ Registered Dietician, nutrition certification, or the ability to obtain upon hire.
+ Experience relevant to the role of the position.
+ Experience/ability to work with a Medicaid funded program.
+ Experience with and understanding of rural, low-income populations.
+ Valid NYS Driver’s License and the ability to meet travel requirements associated with this position.
+ Ability to work flexible hours which may include evenings and/or weekends, as appropriate.
+ Must complete and obtain required nutrition certification.
Preferred Qualifications:
+ Familiarity with Medicaid/1115 waiver implementation
+ Familiarity with reviewing and following recipes and food safety measures in compliance with federal and state guidelines.
+ Demonstrated knowledge of evidence-based food preparation skills.
+ Demonstrated ability to utilize program resources within the general community.
+ Demonstrated ability to plan, teach and evaluate informal educational programs through a variety of delivery methods to reach individuals and groups.

Program Responsibilities:
Program Development - 10%
+ Assist in the development of educational strategies.
+ Assist in the creation of innovative educational programs.
+ Assist in the research, identification and selection of existing resources.
+ Assist in structuring programs to meet the needs of program participants.
+ Assist in the development of programs to meet changing local issues.
+ Assist in utilizing a system of ongoing involvement of participants in the program development process.
Program Delivery, Evaluation and Applied Research - 60%
+ Implement the delivery of screenings, navigation services, enhanced navigation and Nutrition Education individually and as groups to both adults and youth.
+ Implement nutrition educational programing and program activities in support of other Nutrition programming efforts as identified/needed to support the established plan of work.
+ Apply existing subject matter knowledge to create lesson plans.
+ Adapt already established curriculum and materials as needed to meet established program objectives.
+ Provide Nutrition educational programs in the community and via mass media.
+ Work to foster acceptance of the 1115 Medicaid Nutrition Program, methods and policies while addressing community and individual needs and considering diverse audiences.
+ Be involved in multi-association team efforts through joint planning and implementation of multi-disciplinary programs and program activities to deliver the 1115 Medicaid Nutrition Program.
+ Serve as subject matter resource in the area of Nutrition to staff and participants, community organizations and the general public.
+ Serve as a team member and cooperate with the entire Association and Nutrition Program staff to achieve Association program delivery goals.
+ Implement program evaluation as designed.
+ Interact with program participants to obtain evaluation data, to include: evaluating effectiveness of services under the 1115 Medicaid waiver program.
+ Assist Nutrition Program Manager in the process of analyzing program data, and based on that analysis, assist in making recommendations for improvements in program offerings.
+ Communicate evaluation findings to constituents as appropriate.
+ Assist in the design or creation of program evaluation framework.
Administrative Responsibilities:
Direction and Management - 10%
+ Assist in the interpretation of Association program direction to plan and delivery educational programs.
+ Assist in the planning and promotion of major public events and programs including, but not limited to: assisting with mass media efforts related to upcoming events and programs.
+ Assist in identifying and recommending program needs (i.e. program supplies and materials) to the Nutrition Program Manager.
+ Work with Finance Department to assess and to evaluate the overall fee for service components of the program to make adjustments as needed.
+ Assist in solving problems and resolving conflicts that arise in the program utilizing appropriate policies and procedures.
Coordination/Operation - 15%
+ Organize and coordinate program activities related to 1115 Medicaid Waiver Programming (i.e. reimbursable services to adults and to youth etc.).
+ Coordinate identified administrative aspects of the program to include but not limited to: preparing correspondence, maintaining files, monthly reimbursable service(s) tracking, scheduling with participants, etc.
+ Coordinate, compile and organize existing program resources and program materials.
+ Establish and maintain connections between CCE and Medicaid eligible participants.
+ Assist the Nutrition Program Manager in utilizing effective marketing strategies in the promotion of programming efforts to include, but not limited to print media, news media, social media and multi-media efforts.
+ Effectively maintain existing professional relationships with other agencies, organizations and businesses that promote effective communication and facilitate continued programming efforts.
+ This position may require the transport of program participants and/or program materials and resources in performing position responsibilities.
Professional Improvement and Other Duties as Assigned - 5%
+ In cooperation with Nutrition Program Manager, jointly develop and pursue a professional development plan to increase competencies relative to position accountabilities and to address changes in programming and association priorities.
+ Collaborate in activities that are in general support of Cornell Cooperative Extension including but not limited to attendance at staff meetings, timely reporting of expenses, working with other staff, general marketing of Extension programs, serving on organizational committees (as approved by supervisor) and other duties as assigned.
Health and Safety - Applied to all duties and functions.
+ Support the association to maintain a safe working environment to proactively prevent accidents/injuries and when necessary, communicate potential hazards and/or actual incidents to supervisors when they occur in accordance with association policies and reporting practices.
+ Create safe working environments in off-site community locations following all applicable food safety guidelines as well as local partner rules and regulations Report any concerns to supervisor.
+ Be familiar with and strive to follow any applicable federal, state, and local regulations in addition to association health and safety policy/procedure/requirements.
