Job Overview
Coordinates the care of the patient with chronic pancreatitis through all phases’ pre, peri, post.
Responsibilities:
+ Coordinates patient care across the care continuum pre and post-TPIAT for adult patients.
+ Collaborates with MD, interdisciplinary team members, community members, patients, families, and significant others to plan, manage, evaluate care systems and resources.
+ Serves as liaison with physicians to implement medical management of condition changes, diagnostic test results, lab results, based on knowledge of patient and patient condition.
+ Provides triage and makes patient care decisions. Responds to patient phone calls about condition changes. Directs medical care and evaluation as appropriate.
+ Coordinates and identifies the need to include other professional expertise; makes arrangements, both local and national, to include care provides in a multi-disciplinary treatment plan.
+ Provides direct patient assessment and evaluation:
+ obtains patient history
+ provides physician with information on patient status
+ outpatient follow-up visits
+ maintains appropriate records of patient care activities
+ performs assessment of appropriate system function on patients.
+ Acts as liaison with community MD and pharmacists/home care/IV to ensure continuum of care is provided.
+ Meets all role-related and age specific competencies
+ Provides patient, MD, community and professional nurse education.
+ Coordinates and participates in patient, staff, MD and community education.
+ Educates patients on the medical management protocols for necessary compliance.
+ Provides on-going patient education through the life of patient.
+ Provides on-going education to families/S.O. who are in supporting roles to patients.
+ Provides clinical data to local networks and insurance companies as requested.
+ Acts as a role model for other areas of nursing by demonstrating care coordination and arrangement, problem solving skills and efficient utilization of resources across all care settings.
+ Serves as a preceptor in student learning situations.
+ Orients new staff to the role of care coordinator
+ Research.
+ Completes data for FDA reports on + islet cell cultures.
+ Review and respond to patient care issues in returned QOL surveys.
+ Provides program development.
+ Helps develop and maintain clinical care guidelines for the TP-IAT program
+ Develops new approaches to assist patients in management of their health care over the course of a lifetime.
+ Participates in quality improvement initiatives.
+ Serves on committees.
+ Identifies need in assist in writing and implementing new policies.
+ Professional development.
+ Keeps current of new developments in pancreatitis and TPIAT through reading, attending seminars and networking with other coordinators across the nation.
+ Attends, presents and membership in nursing work groups.
+ Demonstrates value and upholds team building principles.
+ Serves as professional role-model.
Required Qualifications
• BS/BS in Nursing
Experience
• 3 years acute care experience in ICU, CCU, Acute Care Med-Surg caring for complex pancreas patients or other relevant nursing experience
License/Certification/Registration
• RN License

Compensation Disclaimer
The posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
