    Description:

    The Quality Improvement Lead assists in leading the Quality Department at Nazareth Hospital (NH) and St. Mary Medical Center (SMMC) campuses to provide a high quality of care for our patients and families utilizing our Quality Metrics (ECQM) cornerstones.

     

    • Registered Nurse with minimum of five years of acute care hospital experience required.

    • Bachelor’s degree preferred in Healthcare Administration, Nursing, Social/Care Management, Business, or related field.

    • Strong interpersonal and communication skills.

    • Demonstrated presentation and facilitation skills.

    • Knowledge of patient satisfaction surveys, performance improvement strategies, and healthcare quality standards.

    • Must be able to work independently and on a team.

    • Knowledge of Microsoft Office, data base management, spreadsheets, internet skills and e-mail.

    The Quality Improvement Lead assists in leading the Quality Department at Nazareth Hospital (NH) and St. Mary Medical Center (SMMC) campuses to provide a high quality of care for our patients and families utilizing our Quality Metrics (ECQM) cornerstones. The Quality Improvement Lead will assess, analyze, and recommend quality and clinical performance improvement processes to assure that the highest standards of quality care can be achieved. Will provide clinical expertise to assist with case-finding, identification of opportunities, and performance improvement activities. The Quality Improvement Lead will engage and collaborate with nursing and physician leaders to improve/correct nursing and physician documentation to ensure eCQM metrics are being met. This position will also measure performance and see opportunities to enhance performance in value-based systems of care. As healthcare evolves with new models of service delivery, quality improvement activities will aim to ensure that care is both patient-centered and focused on maintaining the health of our entire patient population in an efficient and effective way. This position serves as a key participant in the successful management of value-based contracts, CMS ECQM metrics and focusing on both individual patients and population trends. The Quality Improvement Lead collaborates closely with clinical, operational, and administrative teams to promote quality performance, develop action plans and when necessary collaborate with nursing leaders on remediation plans to ensure quality metrics are met and maintained. Quality Improvement Lead will present quality reports to various committees ensuring performance improvement goals. Data collection and analysis skills are integrated to be utilized in the performance improvement (PI) process, as well as other responsibilities of the position. The Quality Improvement Lead will participate or co-chair/facilitate committees/teams as requested by the Quality Manager. The Joint Commission, CMS, and other regulatory bodies including regulatory compliance will be part of the Quality Improvement Lead position.
