    COMMUNITY HEALTH WORKER LEAD

    Boise Area

    POSITION PURPOSE:

    A Lead Community Health Worker (Lead CHW) is a seasoned, frontline public health worker who is a trusted member of and/or has a remarkable understanding of the community served and serves as a support to Community Health Workers.

     

    The Lead CHW coordinates and supervises daily activities of a team of Community Health Workers engaged in providing outreach, education and access to resources. The Lead CHW acts as a link between the Community Health Workers, health system teams, and the community to facilitate access to services and improve the quality and cultural competence of service delivery.

    WHAT YOU WILL DO:

    + Conducts and interprets individual assessments to identify non-clinical needs and mitigate any barriers to resources and services.

    + Improves access to health and human services for individuals through in-person interactions, telephone contacts, and coordination of referrals per protocols of ministry's specific programming.

    + Provides basic education regarding chronic health conditions and health promotion in a way that promotes understanding and self-management by the individual. Reinforces health literacy and educational messages using appropriate tools and supplies within scope of practice.

    + Facilitates or conducts enrollment in available federal, state and local programs to increase access to health care and support services. Examples include health coverage via the insurance marketplace, Medicaid, Medicare, social security benefits, food, and housing services.

    + Serves as a liaison between providers and patients to facilitate communication and coordination of services. Coordinates and monitors services, including comprehensive tracking of patients' adherence in relation to care plan objectives per protocol.

    + Continuously expands knowledge and understanding of community services and resources; maintains frequent contact with community agencies serving vulnerable populations and participates in community meetings.

    + Uses Social Care tools via TogetherCare (or similar protocols) for care coordination.

    + Provides performance feedback to Community Health Worker performance using effective communication.

    + Promotes the services of the Community Health Workers within the health system and community through agency events, speaking engagements, inter-departmental meetings, in-services, local media, and literature distribution.

    + Designs, develops, implements, and evaluates best practices as they apply to Community Health Workers utilization. Identifies training needed by Community Health Workers and develops agenda for training sessions.

    + Works with the supervisors and clinical care teams to establish protocols for Community Health Workers engaging individuals, completing the documentation and working with community resources.

    REQUIREMENTS:

    + High school diploma or GED.

    + Must have at least one year of experience in community involvement, facilitating referrals to health/social services

    + and supports, health system navigation such as applying for insurance and other coverage plans, and/or providing

    + health-related education/intervention to individuals and groups with diverse backgrounds.

    + Demonstrated lived experience (such as experiencing poverty, food insecurity, housing instability, substance use,

    + etc.) and/or cultural background of the patient community in which this position will work in and/or with.

    + Specialized training/certification or demonstrated substantive knowledge in a specialty track and/or evidence-based

    + intervention (e.g., asthma, diabetes, behavioral health, gerontology etc.).
