    Overview:State Street Apartments is a 6-bed, co-ed residential housing unit for homeless young adults with diagnosed mental illness, 18 to 25 years of age. The program offers intensive mental health case management services in a supportive environment. State Street Apartments serves young adults in residence and also those living in the community who are homeless or at imminent risk of becoming homeless. The young adults who are part of this program receive independent living skills training, social group support and educational and recreational services focused on building self-sufficiency.

     

    Case Manager's: In the role of a Case Manager, you will provide individual case management/social work services and independent living skills (ILS) training, in collaboration with the Case Aide, to a minimum of 15 young adults as assigned. CM's also will conduct comprehensive intake, assessment, and evaluations. Create and implement individualized service support plans.

    Responsibilities

    + Initiate and maintain regular contact with assigned clients, conduct home visits and living skills sessions of significant duration to assist them with acquiring skills to meet their goals of self-sufficiency. Frequency determined by client’s needs. Provide therapeutic support and crisis intervention.

    + Maintain statistical accountability and accurate written information for complete case records and provide ongoing assessments and case recommendation. Compile data and complete reports.

    + Collaborate with the Case Aide to arrange deliver of ILS services with other providers, agencies and programs and make referrals for auxiliary services as needed.

    + Participate actively in case conferences, staff meetings, supervision, professional and in-service training sessions. Maintain connections with outside agencies and ensure professional representation of the agency in the community.

    + Fulfill 24 hour on-call responsibilities as assigned.

    Qualifications

    For the position of Case Manager at State Street Apartments, we do look for the following in your credentials:

    + Associates Degree with at least 1-3 years of experience working with young adults.

    + Bachelor’s Degree is preferred but not required.

    + Experience with mental health services and knowledge of community resources.

    + This position will require the employee to drive a Salvation Army vehicle or van.

    + You are required to have a valid NYS Driver License that will meet our Insurance Companies requirements for safe driving.

    + Background Check, Fingerprinting, SCR, and Drug Screen are part of the screening for this position.
