    Basic Function

     

    To assist social work, corrections, or victim/witness staff in arranging for the provision of services to clients by determining program eligibility, opening cases and monitoring case activities, reviewing and processing vendor payment claims, maintaining necessary files and records; informing trial witnesses when needed in court; and perform related duties as assigned.

     

    The Aging & Disability Services | MnCHOICES Division within the Social Services Department is responsible for administering Home and Community Based Services. The division is comprised of three major functional areas: The MnCHOICES unit, Administration & Access unit, and the Case Management unit. It is a priority and expectation of the county and the department to operationalize racial equity across the administration of Home and Community Based Services, as well as grow an organizational culture that is value based: People, Integrity, Community, Equity, and Leadership; and embodies the Mission & Vision of Ramsey County.

     

    The Case Aide position serves in the MnCHOICES Unit – Reassessment Intake or Initial Intake Team. The position provides ongoing support to intake social workers, assessors, case managers, supervisors, and section as needed. Primary responsibilities include but are not limited to: Support assessors with assignment, appointment setting, insurance/financial paperwork tracking, nursing home tracking and in obtaining assessment information; Provide information referral and assistance to people corresponding regarding MnCHOICES assessments; Assist other units in the Section as need arises and work collaboratively with them; Research client information in MMIS and MAXIS; Enter Screening Documents and Service Agreements in MMIS and Assist Assessor with resolving MMIS screen system edits for approval of documents, and when needed communicate with DHS Help Desk; Track cases after assessments to assist with receiving waivered services; Daily documentation of work in appropriate computer system including SSIS, MNCHOICES and MMIS and other systems as appropriate; Provide effective and responsive services across language, culture and race; Work as part of an integrated care team, assisting co-workers and responding to needs of other agency clients; Manage special projects as requested.

     

    This Case Aide position serves in the Admin & Access Department and works with the Community First Services and Supports (CFSS) program. CFSS is a Minnesota health care program that offers flexible options to meet the unique needs of people. CFSS allows people greater independence in their homes and communities. CFSS will replace personal care assistance (PCA) and the Consumer Support Grant (CSG).

     

    The Case Aide job duties include but are not limited to: managing the PCA/CFSS email account to monitor new and renewal Ramsey County residents enrolling in CFSS; Monitor the CFSS phone line to track CFSS inquires and residents CFSS consultation provider selection; Track all new and renewal PCA/CFSS cases in excel spreadsheet in TEAMS and MnCHOICES tool; enter and monitor service agreements in MMIS; work collaboratively with assessors, case manager, PHN vendor agencies, etc. to get residents services in place.

    Flexible Workplace

    This position is identified under the designation of ‘flex work eligible’, meaning that the employee can formally opt to be in-office full-time or work a flex schedule in which at least two-days per week are performed in office and other days can be performed in a remote-first environment. Regardless of selection, the position carries expectations regarding on-site responsibilities and will require schedule flexibility beyond the minimum expectations set forth in the county’s flexible workplace policy.     Examples of Work Performed

     

    + Interview clients in need of vendor provided services such as adult day care services, home health care aide, foster care or day care services, unaccompanied minor or SELF programs; and placement services for juveniles referred through the corrections system.

    + Determine initial and on-going eligibility for services by comparing financial and social background information to program eligibility requirements; and refer clients to other sources for assistance as needed.

    + Open service cases by obtaining necessary intake information from client, computer data bases, case files or other community agencies; prepare intake documents required for initiating or updating case files; and forward case for assignment to staff.

    + Monitor on-going cases by obtaining periodic case update information and entering information on case files; follow up with clients who fail to return completed eligibility documentation; initiate closure of case files for clients who do not meet ongoing eligibility criteria.

    + Review vendor payment claims for accuracy, check claim amounts against established fee schedules, initiate payment documents and maintain payment records.

    + Assist in contacting victims of adult and juvenile probation clients to gather documentation and to determine appropriate amount of restitution to be ordered by the court; maintain records of restitution payments for adult probation clients; accept payments and update restitution schedules as determined by court order.

    + Maintain attendance records for adult probation clients who have been ordered to attend probation programs; monitor schedule of drug screening appointments; prepare documentation of case status for probation officers.

    + Monitor calendar and schedule administrative reviews for foster care cases with Social Workers and other service providers as required by law; may attend administrative reviews to complete necessary case documentation; forward require case status documentation to State human services personnel to fulfill program monitoring requirements.

    + Prepare reports concerning clients' needs and services as requested.

    + In rare occasions, may transport clients to medical appointments, supervised visits, court hearings, placements and other service programs/facilities.

    + Assist social workers in emergency situations by contacting departmental or community resources on behalf of clients; may assist probation staff in conducting on-site visits to group home facilities.

    + Assist victim/witness advocates by checking with court clerks to learn trial line-up, and by checking with attorneys to learn the order of witnesses and communicating to others as necessary;

    + Assist victim/witnesses advocates by keeping law enforcement, professional and occasionally civilian witnesses informed of when they will be needed in court to testify; confirming their arrival; updating others on status; informing witnesses when they are no longer required at court.

    Minimum Qualifications

    Education: High school graduation or equivalent.

     

    Experience: Four years of clerical experience.

     

    Substitution : Equivalent combination of education and related experience.

     

    Exam/Screening Process Information

    The examination process will consist of the following section with each section weighted as indicated:

    + Training and Experience Rating = 100%

     

    The examination for case aide will consist of a training and experience rating, comprised of the questions in the attached supplemental questionnaire. Not all applicants who meet the minimum qualifications will pass the training and experience rating. The rating on this supplemental questionnaire will depend on your answers to the questions – do not mark “see resume” or “see work history” Do not cut and paste resume as a response. Please answer all of the supplemental questions clearly and completely, failure to do so may affect your ability to move forward in the hiring process.

    Eligible List:

    The names of all applicants who have submitted a completed application and passed the examination/screening process shall be placed on the eligible list for an employment opportunity as a case aide. This list will be certified to the appointing authority which may use this list to conduct interviews to fill a vacancy. Applicants will remain on the list for 60 days or until hired, whichever occurs first. A notice will be sent to applicants at the time the eligible list is posted.

     

    Veteran's Preference: This is a classified position requiring an open, competitive selection process. Veteran's Preference points will be applied after a candidate passes the examination process. Applicants who are eligible for veteran's preference should update their veteran's DD214, and other supporting documents, and submit them as an attachment at the time of application.
