**Summary:** The Director of Continuing Care Services Business Development oversees operations and programs for Home Health, Hospice Coordinators, and Home Care Specialists. Responsibilities include managing budgets, Central Intake, and clinical/non-clinical teams across multiple hospitals, with a focus on departmental growth, teamwork, and high-quality patient care.
Responsibilities
+ Continuing Care Services Operations- Plays a key role in supporting throughput and facilitating timely discharges to care at home by coordinating with hospital teams, ensuring appropriate resources are in place, and streamlining transitions for patients requiring home-based care services. Collaborates with interdisciplinary teams in each department to plan for and address issues with changing patient care delivery system and services. Provides guidance and follow-up with the team to ensure that any changes are implemented as planned. Develops and implements standards of practice within areas of responsibility that enhance quality of services provided and ensures compliance with regulatory agencies by remaining current on these regulations and informing others of updates and/or changes needed. Collaborates with clinical managers in overseeing clinical teams and in planning for area(s) of responsibility. Guides and counsels coordinators and Department Directors to assist them in improving all aspects of home health, home care or hospice services provided through the staff.
+ Collaboration: Collaborates closely with Case Managers and Clinical Supervisors within the Hospitals to establish positive working relationships, educate on regulatory changes occurring in CCS departments, Market all services within the hospital to case management, hospitalists and Clinical Managers providing data regarding the ability of CCS to provide high quality care to patients in the community.

Education Requirements:
+ Bachelor’s degree in an appropriate discipline.
Licensure/Certification Requirements:
+ Clinical license preferred. (either as PT, OT, or RN) by the State of North Carolina.
Professional Experience Requirements:
+ Four (4) years of clinical experience, including two (2) years in home health management/administration or hospice management/administration.
Knowledge/Skills/and Abilities** **Requirements:
+ Understands and applies principles, procedures, requirements, regulations, and policies related to specialized expertise.
