What You’ll Do

+ Deliver high-quality physical therapy services in a home setting

+ Conduct assessments and develop individualized care plans

+ Educate patients and caregivers on exercises and mobility techniques

+ Collaborate with interdisciplinary teams to ensure holistic care

+ Use advanced, user-friendly technology to document and track progress

What You Bring

+ Degree from an APTA-accredited physical therapy program

+ Active state license to practice as a Physical Therapist

+ At least 1 year of experience or completion of a 6–8 week preceptorship

+ Home health experience and knowledge of Medicare Conditions of Participation preferred

+ Strong organizational, communication, and tech skills

+ Valid driver’s license and reliable transportation
