    + **Shift Hours:** Days Varies, 8:00 a.m. - 5:00 p.m.

    + **Weekend Work:** Occasional

    + **On Call:** No

    + **Home visits in assigned service area:** Dunedin, Clearwater, Palm Harbor, Oldsmar, and surrounding areas

     

    The **Home Health Aide** - **Per Visit** provides **hands-on care** in the home for a group of patients consistently using the nursing process and clinical standards and protocols. **Responsibilities include** :

     

    + Working under the guidance and supervision of the staff nurse in providing personal care to the patients

    + Following the patients' plan of care for completion of tasks assigned to a home health aide by the registered nurse or other appropriate skilled professional

    + Maintaining an open communication process with the patient, representative (if any), caregivers, and family

    + Demonstrating competency with assigned tasks; complies with infection prevention and control policies and procedures; reports changes in the patient condition; and honors patient rights

    Home Health Aide - Home Care Requirements:

    + Driver's License State of Florida (Florida driver's license is required within 30 days of hire)

    + CNA (Certified Nursing Assistant) or HHA certification from accredited 75-hour program

    + BLS (Basic Life Support)

    + **Preferred:** High School Diploma or equivalent

    + Preferred 1 year Home Care
