Logan Health has a collection of services and specialists who are committed to excellence in education, treatment, and care of our patients.• Seeking part-time Licensed APP to support the development and expansion of our Dementia Program• Minimum of 1–2 years APP experience preferred• Experience with geriatric or memory care populations strongly preferred• Outpatient-based care• No call responsibilities• Travel required to patient homes within the Flathead Valley, as needed• Some telehealth services may be incorporated• Collaborative team approach – working closely with a Nurse Navigator and Medical Assistant
