Job Title: Service Coordinator, Help Me Grow
Unit: HMGBF Expansion
Shift: Mon-Fri, 8a-4:30p

Position Summary:
The Service Coordinator/Home Visitor is responsible for providing comprehensive support, education, and service coordination to clients and their families through home visits. This position aims to promote family well-being, child development, and access to community resources to empower pregnant people and families with young children to thrive. Responsibilities include:
• Home Visiting: Conduct regular home visits to assess family needs, support parenting practices, monitor child development, and engage families in evidence-based curriculum.
• Service Coordination: Develop, implement, and monitor individualized service plans based on family goals and needs and support and/or facilitate transition to preschool services.
• Resource Referral & Coordination: Connect families with appropriate community resources such as healthcare, housing, employment, education, and mental health services.
• Documentation & Reporting: Maintain accurate and timely records of all interactions, assessments, referrals, and follow-ups in accordance with program guidelines.
• Advocacy & Support: Advocate on behalf of families to ensure access to services and promote self-sufficiency and empowerment.
• Program Compliance: Ensure services are delivered in alignment with agency policies, funding requirements, and applicable regulations.
• Family Engagement: Foster strong, supportive relationships with families to encourage ongoing participation and engagement in services and child development activities.
• Team Collaboration: Work closely with multidisciplinary teams (educators, health professionals, supervisors) to support coordinated service delivery.
The Service Coordinator/Home Visitor reports to the Home Visit Coordinator for Ongoing Services. The SC/HV is accountable for compliance with all applicable regulatory standards, policies, procedures, and protocols of the
Human Resources Department, the Greater Dayton Area Hospital Association (GDAHA) and any funding sources that support the work of the department.

Education:
Minimum Level of Education Required: Bachelor's degree
Additional requirements:
• Type of degree: Bachelor's degree
• Area of study or major: Early Childhood or related field. If unrelated field, then child development experience required.
• Preferred educational qualifications: Early Childhood, Teaching, Social Work, or Nursing degrees or related field
• Position specific testing requirement: Must undergo a criminal background check with the Ohio bureau of criminal identification and investigation and a background check with the Ohio's Statewide Automated Child Welfare Information System (SACWIS) Registry on Child Abuse and Neglect.

Experience:
Minimum Level of Experience Required: 1 - 3 years of job-related experience
Prior job title or occupational experience: Minimum two years’ experience with young children and their families if degree is not in Early Childhood, teaching, social work, or nursing. Prefer home-visiting program experience. Demonstrates a thorough knowledge of and the ability to work with community resources appropriate to the needs of at-risk populations.
Prior specific functional responsibilities: NA
Preferred experience: At least 1 year working with pregnant women, infants, and children.
Other experience requirements: Upon hire, complete training and certification per the Help Me Grow guidelines.

9. Flexibility and adaptability in response to changing family needs and service environments.
Requirements Include:
1. Possession of a valid driver’s license, reliable transportation, and current auto insurance.
2. Must adhere to agency policies regarding tobacco use, including maintaining a smoke-free presence during work hours and ensuring that clothing and personal items are free from the smell of tobacco.
