    Telephone Triage NurseJob Description

     

    This role involves supporting patients transitioning between various care settings, ensuring continuity and quality outcomes. The nurse will collaborate with care teams, patients, and families to coordinate resources, manage care plans, and optimize recovery using clinical expertise and motivational coaching.

    Responsibilities

    + Coordinate care transitions across inpatient, outpatient, and home settings.

    + Conduct patient and family consultations to support care planning.

    + Facilitate discharge planning and follow-up care.

    + Collaborate with PCPs and interdisciplinary teams.

    + Monitor patient progress and promote self-management.

    + Document care activities and maintain compliance with protocols.

    Essential Skills

    + Active and unrestricted RN license in Florida; BSN preferred (ASN with 4+ years accepted).

    + Minimum of 2 years clinical experience in a healthcare setting.

    + Valid FL driver’s license and reliable transportation.

    + Strong communication, organizational, and decision-making skills.

    + Proficient in electronic health records and HIPAA compliance.

    Additional Skills & Qualifications

    + Experience in care management, utilization review, or discharge planning.

    + HMO experience preferred.

    + CCM certification is a plus.

    Pay and Benefits

    The pay range for this position is $40.00 - $45.00/hr.
