    Your Role

     

    As a Physical Therapist, you’ll deliver professional, evidence-based care in accordance with physician orders, state regulations, and agency policies. You’ll help clients regain independence and improve their quality of life through personalized rehabilitation plans.

    Key Responsibilities

    + Conduct comprehensive assessments and develop individualized treatment plans.

    + Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.

    + Educate clients, families, and caregivers on therapy techniques and home programs.

    + Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).

    + Collaborate with interdisciplinary teams and participate in care conferences.

    + Monitor progress, adjust treatment plans, and communicate changes to physicians.

    + Maintain accurate, timely documentation in compliance with regulations.

    Qualifications

    + Degree from an APTA-accredited Physical Therapy program

    + Active state license to practice as a Physical Therapist

    + 1+ year of experience or completion of a 6–8 week preceptorship

    + Home health experience and knowledge of Medicare Conditions of Participation preferred

    + Valid driver’s license and reliable transportation

    + Strong organizational and time-management skills

    Physical & Mental Requirements

    + Ability to lift up to 35 lbs and assist with patient mobility

    + Frequent standing, walking, bending, and positioning

    + Adaptability to changing schedules and environments

    + Travel to client homes and Trinity Health locations required
