    **Job Title:** Physical Therapist

    **Ministry Name:** Saint Joseph PACE

    **Hours:** Monday-Friday from 8:00 am – 4:30 pm. There is no evening, on call, or weekend hours.

     

    Sign On Bonus $7,500

    Position Purpose:

    Join our compassionate, mission-driven team as a Physical Therapist to provide clinical evaluation and treatment for participants across Day Center, nursing home, and in-home settings. You'll support seniors in maintaining or regaining independence, ensuring care is personalized and coordinated within an interdisciplinary team.

    What You Will Do:

    + Perform in-person physical therapy assessments and develop individualized care plans.

    + Provide restorative and maintenance therapy to frail or elderly participants.

    + Supervise and mentor Physical Therapy Assistants.

    + Collaborate with nursing homes, assisted living, and interdisciplinary teams.

    + Recommend and evaluate durable medical equipment, braces, prosthetics, and safety devices.

    + Maintain compliance with all federal, state, and internal policies.

    + Participate in staff training, safety initiatives, and quality assurance programs.

    Minimum Qualifications:

    + Bachelor’s degree in Physical Therapy from an accredited program.

    + Active, unencumbered Physical Therapy license in the state of practice.

    + At least 1 year of experience with elderly or frail populations (or willingness to be trained).

    + Strong communication, organizational, and documentation skills.

    + CPR and First Aid Certification (as required).
