Clinical Admissions Facilitator-Full-Time-in SYRACUSE, New York for Sunnyview Rehab Hospital
Requires:
NYS Physical Therapist license

What you will do:
Sunnyview Rehab Hospital a premier physical rehabilitation hospital located in Schenectady, NY, is in need of a Clinical Admissions Facilitator **working in Syracuse, NY** to perform preadmission screenings, which is a detailed and comprehensive clinical assessment of the patient's condition, need for rehabilitation therapy and medical treatment to be provided in the Inpatient Rehabilitation Facility (IRF) setting. Incumbent will be working out of their home based in Western, New York and traveling to area hospitals when needed. This position is Monday through Friday business hours with occasional weekend and holiday coverage as needed. **This position will work out of St. Joseph's Hospital, Crouse Hospital, and Upstate University Hospital,** screening for Sunnyview Rehab Hospital.
Responsibilities:
+ The Clinical Pre-Admission Facilitator is responsible for patient assessment for acute rehabilitation based on established criteria of the acute rehabilitation center and with compliance to CMS regulations.
+ Education of clients, family members, referral sources, and external payers regarding the effective and efficient utilization of program services and available resources.
+ Screening of clients through on site and/or telephonic assessment and record review in collaboration with physicians, nurses, therapists, social workers, case managers and other interdisciplinary team members involved in the client's care. If the client is unable to participate in an interview, family members or significant others may also be interviewed to obtain accurate information as to pre-morbid functional status, personal interests, cultural considerations, and discharge plans affecting the rehabilitation admission.
+ Determination of medical stability, readiness for program participation, and those services recommended are appropriate, reasonable and necessary. This is done in collaboration with the physical medicine and rehabilitation physician or other licensed treating physician with specialized training and experience in rehabilitation.
+ Facilitation of benefit eligibility and precertification by payers prior to the rehabilitation admission.
+ Coordination of a smooth transition from the referring facility or site to the rehabilitation setting
+ Performs on-site and/or record review, including telephone preadmission screening and clinical assessment
What you will need:
+ Must have Active Licensure as a Physical Therapist, Occupational Therapist, or Registered Nurse
+ A minimum of **2 years of clinical experience** ; experience in the rehabilitation of chronically or catastrophically ill or injured individuals or in an acute care hospital required
+ Acute Rehab knowledge is a MUST
+ Demonstrated expertise in analysis, decision making, time management, oral and written communication, and computer use
+ Excellent assessment skills and knowledge of insurance and managed care principles and practices
+ Ability to function independently
+ The position requires a moderate amount of physical work - individual must be able to maneuver throughout halls, stairways, and patient rooms. Interpretation of environmental input requires visual and auditory skills
+ Ability to utilize multiple Electronic Medical Record Platforms
_Pay Range: $35.80-$48.10 Pay is based on_ **_license type,_** _experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location_
