The Idaho Division of Veterans Services (IDVS) is recruiting for a part-time Social Worker (28 hours per week) for the Veterans home in Pocatello!

The ideal candidate for this position will be relied on for providing professional social work services to elderly residents and their families.

Responsibilities:

+ Provides professional social work/case management services to residents and families.

+ Conducts psychosocial assessments for the purpose of admission screening and evaluation of treatment and progress in the applicant's immediate milieu.

+ Participates in interdisciplinary team staffing to formulate treatment plans.

+ Provides individual casework counseling and evaluation services to implement specified treatment plans.

+ Provides intervention services to residents and their families throughout the residents' stay at the facility.

+ Communicates with relatives, guardians, and other appropriate persons regarding the resident's need for assistance in adjusting, modifying, or improving social functioning and behavior.

+ Investigates and resolves complaints made by or on behalf of the residents.

+ Represents residents on behalf of their claims, expenses, and their requests to the facility.

+ Inputs data with subsequent updates into the MDS Plan of Care for nursing care residents.

Minimum Qualifications:

+ Licensure as a Social Worker in Idaho in accordance with Idaho Code 54-32.

MQ Specialties:

+ Experience in a health care setting or facility social service.

Desired Qualifications:

+ Knowledge of the Minimum Data Set (MDS) process.
