•‍⚕️ What You’ll Do as a Pediatric RN Case Manager:

+ **Coordinate Pediatric Home Care** – Oversee individualized care plans for children with acute and chronic conditions

+ **Deliver Direct Nursing Care** – Administer meds, manage wounds, trachs, vents, and respond to clinical needs

+ **Educate & Empower Families** – Provide instruction and emotional support to caregivers and families

+ **Collaborate with a Care Team** – Work closely with physicians, therapists, and internal staff to ensure quality outcomes

+ **Monitor, Document & Adjust** – Track progress, respond to changes in condition, and maintain clear clinical records

+ **Lead with Compassion** – Serve as an advocate and champion for your patients and their families

• What You’ll Need:

+ 🎓 **Graduate** of an accredited nursing program

+ 🧸 **At least 1 year of RN experience** , with pediatric care strongly preferred

+ 🏥 **Active RN license** in the state of Connecticut

+ 🚗 **Reliable transportation** , valid driver’s license, and auto insurance

+ 💪 Ability to lift up to 100 lbs. and perform physical nursing tasks

+ 💡 Strong clinical judgment, communication skills, and a heart for service
