We’re currently seeking a **skilled and compassionate After Hours/On-Call RN** to join our **Hospice Team** . You’ll play a critical role in delivering high-quality care during evenings, weekends, and holidays — ensuring that our patients and families always have the support they need.

•⚕️** **_What You’ll Do as an After Hours/On-Call RN:_
+ Provide compassionate and timely hospice care to patients and their families outside regular business hours
+ Assess patient physical, psychosocial, and spiritual needs and document findings in clinical records
+ Collaborate with the interdisciplinary team, physicians, and family members to deliver care aligned with the hospice plan
+ Educate patients, families, and facility staff on pain/symptom management and hospice philosophy
+ Offer emotional and bereavement support at the time of death
+ Escalate critical updates to the clinical supervisor and participate in ongoing care evaluations
+ Travel locally to patient homes or facilities (nursing homes, assisted living, hospitals, etc.)
•** **_What You Bring to the Role:_
+ Graduate of an accredited RN program (ADN, Diploma, BSN, or equivalent)
+ Current RN license in [Insert State]
+ **Minimum 1 year** of clinical nursing experience (hospice/home health preferred)
+ Compassionate communication skills and sound judgment under pressure
+ Reliable transportation, valid driver’s license, and auto insurance
+ Ability to lift 50–100 lbs and work variable schedules, including weekends and holidays
+ Comfortable with 50% local travel
