    Pine Acres Rehabilitation & Care Center (Nebraska City, NE)


    Responsibilities include:

    + Complete the nursing assessment and the Baseline Care Plan.

    + Participate on a continuing basis in the development and revision of written care plans for residents as outlined in Prestige nursing procedures. Ensures that documentation meets Federal, State, and Certification guidelines.

    + Coordinate RAI process assuring accuracy, timeliness, and completeness of MDS, CAA, and Comprehensive Care Plan.

    + Conduct the nursing process – Assessment, Diagnosis (actual or potential health problems), Planning, Implementation and Evaluation – under the state’s Nurse Practice Act for Registered Nurse Licensure.

    + Participate in oversight, review, and improvement plan of Quality Measures.
