Position Summary
The Community Navigator works in collaboration with physicians, APC's, behavioral health consultants, care coordinators, RN's, LPN's, and Medical Assistants within the office and is an integral part of the care team for patients.
The community navigator will contact/advise patients, regarding general health/wellness, diagnostic screenings, and follow-up, especially high-risk and vulnerable populations.
The community navigator works as an advocate for patients by empowering them to access community resources through the provisions of such services as education, role modeling, outreach, or referrals.
The community navigator works closely with medical providers to help ensure that patients have comprehensive and coordinated care and work cooperatively with other clinical personnel who are assigned to the same patient.
The community navigator works proactively with high no show patients and patients who have high emergency room use and continues with assistance in the community to assure proper linkages for needs and to assure success.
Must be knowledgeable about community resources, quality metrics, and patient engagement appropriate to needs of patients / families.
Must provide consistent communication to the care team.
This position will not be limited to covering one office but will support multiple Mercy offices.
Essential Job Functions
+ Establish trusting relationships with patients and their families while providing general support, assuring insurance coverage is established and other needs linked while providing excellent working relationships with patients, visitors, and staff; effectively communicating Mercy's mission
+ Assist multiple Mercy Clinics in their achievement of MaGiCQ (PC6) Core Quality Measures
+ Assist multiple Mercy Clinics in follow up on Social Determinants of Health Screening results - ensuring patients are connected with appropriate resources
+ Assist multiple Mercy Clinics in working Care Everywhere reporting in CarePath
+ Work proactively with no show patients and patients with high emergency room use
+ Become adept at using EMR and documenting appropriately in EMR
Employment Qualifications
+ **Required Minimum Education:** Vocational/Technical Degree
+ **Preferred Licensure/Certification:** Community Health Worker Certification
+ **Knowledge, Skills and Abilities:** Experience with Microsoft Office: Word, Excel, and PowerPoint
