As a Social Worker with WesleyLife At Home, you will:

+ Explain WesleyLife Hospice services to patients and families.

+ Visit patient homes (wherever they call home) to assess the psychosocial status, safety concerns, and bereavement risk factors of patients and families/caregivers.

+ Communicate findings to patient care teams to update care plans.

+ Carry out social evaluations and plan interventions.

+ Counsel patients and families/caregivers regarding stress and other identified coping difficulties.

+ Provide information and referral services for patients and families/caregivers regarding financial, practical, and environmental needs.

+ Serve as liaison with patients and families/caregivers and care providers or community agencies.

+ Participate in caregiver, placement, and discharge planning when needed, including discharge to a higher level of care.

Qualifications

Licenses & Certifications

Required

+ Driver's License

+ Social Worker

Experience

Required

+ Valid driver's license, reliable transportation and current auto insurance

+ 3+ years of previous social work experience

+ Master of Social Work (MSW) is preferred

Preferred

+ Previous home health or hospice experience is highly preferred
