    JOB TITLE: SUD Therapist- Per Diem/Part Time

     

    POSITION GRADE: 82 [ salary range: 22-24hr commensurate with education and experience ]

     

    DEPARTMENT/PROGRAM: LIFE House

     

    SUPERVISOR: Clinical Operations Manager

    JOB SUMMARY:

    Under the general direction of the Clinical Operations Manager, the LIFE House Therapist is responsible for providing individual, group and family therapy. The LIFE House Therapist will also be responsible for the maintenance of client records in compliance with oversight agencies and agency policies. This individual will carry out all job responsibilities with fidelity and accountability to the Agencies Mission, Vision, Values, Guiding Principles and Strategic Plan and commitments to the Sanctuary Model of Care.

    ESSENTIAL FUNCTIONS:

    + Exercise full compliance with the Agency’s Code of Conduct, all Agency Policies and Procedures.

    + Maintain confidentiality and security for all client and staff related materials and/or records.

    + Demonstrates, promotes and practices Diversity, Equity, Inclusion and Belonging in relation to clients and staff through respect and understanding achieved through training, individual supervision and Agency cultural activities.

    + Provides crisis intervention services for individuals experiencing acute distress.

    + Engages in the milieu setting with clients for purposes of group therapy, rapport building and recreational activities.

    + Conducts intake assessments and renders diagnoses of individuals with substance use/mental health disorders.

    + Utilizes evidence-based behavioral health therapy interventions for providing individual, group, and family therapy sessions, as prescribed in recovery plan.

    + Develops an individualized service plans in collaboration with the client; and parent/guardian/significant others as appropriate, based on needs identified in assessment. Complete service plan reviews with the client.

    + Provides family counseling and educational sessions to parents, siblings and significant others regarding understanding of the mental health/substance use diagnosis, symptom management and interventions.

    + Assists in developing a comprehensive discharge plan for client that assures their ongoing treatment needs are met after discharge.

    + Complete discharge summaries and coordinate transition planning.

    + Travels as needed in the community to promote collaborative relationships and client centered care.

    + Collaborates regularly with outpatient treatment providers, school staff, referral sources, community providers, and other collateral contacts.

    + Connect clients to aftercare and community based substance use/mental health treatment, and other services as indicated.

    + Seeks clinical supervision to develop competencies and skills in areas of professional development.

    + Responsible for the maintenance of client records in compliance with Office of Addictions Services and Supports (OASAS) regulations to include, but not limited to: Initial assessment, diagnosis, treatment plans, treatment plan reviews, progress notes (including individual, group and family sessions) and all admission and discharge paperwork.

    + Operates independently-uses independent judgement and discretion for the majority of the time.

    + May be responsible for mentoring or supervising interns

    OTHER RESPONSIBLITIES:

    Responsible for travel in the community when necessary with the use of personal vehicle.

     

    + Treatment of clients at other villa of hope outpatient clinic locations may be required as needed.

    MINIMUM QUALIFICATIONS:

    + Minimum of Associates degree required. Bachelor’s degree preferred. Minimum of CASAC required. At least one year of human service experience providing individual, group and family therapy to individuals required.

    + Specialization in the area of addiction and co-occurring disorders preferred.

    + Computer skills, organizational and problem solving skills required.

    + Excellent interpersonal skills and ability to collaborate with others required.
